Dear Ali Reza Ahmadi,

I hope everything is going well

FYI, & follow up we had joint monitoring/supportive supervision from the
Qarabagh district hospital, the administration and nutrition management of
both programs (OPD-SAM & IP-SAM) was in a very bad state, which requires
the serious attention of the implementing organization to take action on
the findings of our supervision. It is as follows:

1. RUTF was not distributed and maintained through the pharmacy system

2. Even though there were 2 cartons of RUTF in the nutrition nurse room,
patients were not accepted, they said that for patients we don't have any
RUTF in stock

3. The stock card was used but not updated

4. The treatment card, follow-up card, and register were not filled
correctly, which means:

(The treatment card was left blank for 2-3 weeks and no action was taken,
the contact number of any of the patients was not registered, until based
on that we visited the relevant family at the community level, and the next
appointment was not clear for the majority of the patients, in some of the
treatment cards numbers of RUTF was not determined, e.g we asked from 2 of
the existing patients how many pocket RUTF do you receive weekly based the
number they received was less than that was written in the treatment card.
and antibiotics were not recorded in all cards)

5. Three patients with severe acute malnutrition were hospitalized in IPD
they have TSS & MSS files recommended for milk and medicine, but the
patients were not available (imaginary).

6. We introduced MAM treatment with RUTF instead of RUSF, but they didn't
have RUTF in stock.
