Dear Dr Sahib Tabesh,

In order to make the AFIAT activities sustainable and be part of the HF rou=
tine activities in the future, AFIAT decided some actvitie one of them is t=
o perform a self-assessment by HQIP check lists as the following steps:

*   The Kabul AFIAT PO team will distribute the HQIP checklists within th=
e current week and will conduct a meeting with the HFs staff on how to do t=
he self-assessment
  *   The head of the HF will give the checklists to the HFs staff and the =
HF staff will read the HQIP checklists carefully and will start self-assess=
ment
  *   The HF head will have regular meetings with the HF staff and will mon=
itor and follow up the self-assessment process
  *   The thematic area AFIAT PIOs and CHOs will contact the HF relevant st=
aff and monitor the self-assessment process through phone call
  *   Dr.Habibi will follow the self-assessment process with the HF head th=
rough phone call.

*   When the AFIAT teams go to mentorships to the HFs, the HF staff would=
 have already filled in the HQIP checklists and the AFIAT officer will fina=
lize it and collect the checklists from the HF
  *
  *   Therefore, AFIAT is kindly requesting you to issue a letter to the HF=
s that the HFs strongly support this self-assessment activity. We will not =
bother the HFs routine activities and it is decided that this process will =
take around two months (Nov and Dec 2022)
  *   Note: The AFIAT teams will start the visit of the HFs on Nov 8th so i=
t will be very efficient for the activity's sustainability, if the JACK res=
pected officers also accompany the AFIAT teams in the month of Nov 2022. I =
will share with you the plan by today or tomorrow.
