I have encountered the same issue today in a Chrome browser on a device that normally works fine for Enketo forms.

The fix that worked for me was to reload by typing **CTRL+F5** (I didn't need to go into the settings to fully clear the cache).  Normal refresh didn't work, only the hard reload by CTRL+F5.
