assessment for functionality and unavailability of equipment was done =

Last week I had supportive supervision visits from Waki Khas SHC related =
to Sarubi District.

Below are the main findings which need your kind consideration and =
urgent follow-up.

District: Surobi=20

HF: Waki Khas SHC

Date of Visit:20 August 2022=20

The main findings:

=EF=BF=BD =EF=BF=BD

1- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD  =EF=BF=BDThe nutrition =
counselor was present, but she did not work based on their TOR.

2- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD The GMP and screening were =
not properly done for the entire under 5 children.

3- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD She had the GMP and Screening =
register book but it was not filled by her.=20

4- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD The norms for the detection =
of SAM cases (MUAC, Z-Score, Edema) were not properly followed.

5- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD Poor stock or storage place =
for RUTF.

6- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD Poor documentation.

7- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD Unavailability of the ICE =
material.

8- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD Appetite tests were not done =
for the OPDSAM patient.

Recommended action:

1- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD NC needs refresher training.

2- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD Follow up supportive =
supervision visits.=20

3- =EF=BF=BD =EF=BF=BD =EF=BF=BD =EF=BF=BD Provide the ICE material.=20

During this visit, on-the-job training was done, I worked with them on =
all affected parts and mentioned some action points to the HF staff for =
