Dear Respected colleagues,

Greetings!

Last week I had supportive supervision visits from Waki Khas SHC related to
Sarubi District.

Below are the main findings which need your kind consideration and urgent
follow-up.

*District: Surobi *

*HF: Waki Khas SHC*

*Code:2922*

*Date of Visit:20 August 2022 *

*The main findings:*

1-      The nutrition counselor was present, but she did not work based on
their TOR.

2-     The GMP and screening were not properly done for the entire under 5
children.

3-     She had the GMP and Screening register book but it was not filled by
her.

4-     The norms for the detection of SAM cases (MUAC, Z-Score, Edema) were
not properly followed.

5-     Poor stock or storage place for RUTF.

6-     Poor documentation.

7-     Unavailability of the ICE material.

8-     Appetite tests were not done for the OPDSAM patient.

*Recommended action:*

1-     NC needs refresher training.

2-     Follow up supportive supervision visits.

3-     Provide the ICE material.

During this visit, on-the-job training was done, I worked with them on all
affected parts and mentioned some action points to the HF staff for further
improvement of the program.
