Subject: UN-TPM / Red Flag /Kabul- 1674-Company BHC / HF head is not =
signing forms of data collected

Dear UNICEF Colleagues,

We'd like to report the following red flag.

Province: Kabul
HF: 1674-Company BHC

Issue:
Our team completed data collection in 1674-Company BHC but HF staff did =
not sign the hard copies with summary of data collected.
Based on feedback from our field team, the HF head made a request of =
data manipulation; with the reason that they do not accept the variance =
of verified cases compared to reported cases.

Please do instruct Kabul SP for better assistance with TPM officers in =
the field and confirm data as collected as per the registers (or to =
provide proof why data collected would not be correct).
