Hope you are fine and doing well,

Yesterday I had a monitoring/supportive supervision from Khowaja Jam BHC =
related to Paghman district, generally nutrition programs (GMP/IYCF =
counselling, screening and OPD SAM) were implemented well  but there =
were some weak points that are mentioned as below:

1-     RUTF stock out: RUTF of the HF has finished two weeks ago, and =
follow up cases of OPD SAM are more than 10 cases from the last months, =
the new cases didn=E2=80=99t admit due to lack of RUTF and insufficient =
supply. HF receives 4 cartons (600 sachet RUTF monthly) from the quota =
of Shikhan HF. During the last 3 months there were 2 times RUTF stock =
out. As a result, the OPD SAM program doesn=E2=80=99t implement properly =
based on protocol.

2-     Head of HF, Midwife which is responsible for OPD SAM program and =
NC didn=E2=80=99t receive IMAM and nutrition counselling initial or =
refresher training.

3-     HF didn=E2=80=99t have a digital/Secca scale and also there is =
one measuring board for measuring length of children which is very old =
and should be changed, there is no measuring board for height of =
children.

4-     Admission and discharge criteria in the OPD SAM program =
didn=E2=80=99t follow according to protocol.

5-     OPD SAM children didn=E2=80=99t receive antibiotics at first =
visit (at admission).

6-     Decimal points during measuring height/length and weight of =
children didn=E2=80=99t recorded in GM and OPD SAM register books.

7-     Lack of IEC material (IMAM and MIYCN job aids) in the HF.

8-     Poor IYCF counselling, and screening of children.

Some of the problems were solved in this visit and we hope to follow and =
support other issues which are related to your respected organization. =
Please find attached soft copy of monitoring findings from two HFs =
(Shikhan and Khowaja Jam BHCs) for your kind review and follow up.
