Greetings:
First of all I would like to thank you and your team for the continuous =
follow up of the challenges faced in the provision of quality nutrition =
services in Parwan province. Based on your email and findings of =
monitoring team, JACK Parwan provincial office conducted assessment of =
the situation and provided the following response:
1. The OPD SAM report of Togberdi and Sardara health centers, =
which was not shared before, was received by phone and entered in the =
relevant database, and the officials of these health centers were also =
reminded that in the future they should send their reports to the =
provincial office on time.
2. The Measuring board has been supplied for all the health =
centers mentioned in the feedback.
3. The scales (SECA) have been supplied for all these health =
centers mentioned in the feedback.
4. The position of nutrition councilor in the Ostama SHC, which =
was announced twice, but due to the unavailability of suitable person =
for the mentioned position was delayed and the position will be filled =
very soon .=20
5. The treatment cards of the patients in the OPD SAM Section were =
already printed, but due to the problems in printing of OPD SAM =
registers, these registers were sent to be printed again and =
fortunately the printing of these registers has been completed =
yesterday and will be supplied to health centers by today.
6. The problems related to Babar Health Center, which was =
mentioned in the feedback, was completely addressed .

Some one confirmed from the JACK/Parwan that these HF are missing =

get any response from you to follow the issue.

Thanks for your cooperation and taking action for better results, but =
there are still some problems which need to be corrected ASAP.
I listed below HFs(SHCs) that after 5months IMAM Scale-up, still haven't =
started the OPD-SAM services according to the IMAM Scale-up program. =
those reasons are:
1-Nurtition Counsellor didn't hire.
2-RUTF didn't supply.
3-SECA and Measuring board didn't supply.
4-Register book for OPD-SAM didn't supply.
5-Treatment card for OPD-SAM didn't supply.
Although we are witness that there are enough amount of RUTF, SECA and =
Measuring board at JACK/Parwan warehouse.

3.	Mazana SHC
4.	Toghbirdi SHC
5.	Mazana SHC
6.	Astma SHC

Note: Today we went to JACK for monthly data entry, unfortunately after =
