Dear Respected colleagues and Parwani Sb,

Greetings!

Yesterday I monitored the BHC clinic of Jagdalak in Sarobi District. The =
nutrition program was not properly implementing in this clinic, the same =
practice as which was in the Qamari BHC; the main finding as mentioned =
below:

The nutrition counselor was male; and he was present on their job, but =
he was not working based on their TOR.

They did not have a measuring board for height, and the nutrition =
program had been started in March.

They did not have the SECKA Scale for weight. Only a small digital scale =
for under 2 years=E2=80=99 children was available.

They did not have the GMP and screening register book, and also, they =
did not have the data for GMP and screening.

The GMP and screening were not done for under 5 children.

They had only one register for SAM cases, and they were only taking =
those SAM patients which were referred by CHWs and from OPD of the =
clinic.

The RUTF was not distributed through Pharmacy.

Patient flow did not follow properly, which means that; those under five =
clients or PLWs who were visiting the clinic (midwifery, Vaccinator, and =
doctor) were not referring to the counselor room for screening and =
counseling.

They were distributing 10 IFA tablets to the PLWs. =20

The implementer partner for this clinic is Intersos.=20

I have done on-the-job training and clarified the rules and regulations =
for the SAM program to the clinic staff, and they were promised that =
after this they will follow the instructions.

Also, I have a suggestion for further improvement of the program; we =
need to set a meeting with IP to introduce the standards of the SAM =
program because this is the second clinic that I visited, and they have =
the same practice.
