Despite significant investments in the immunization program, lower =
routine immunization coverage ( ~60% for Penta3_AHS2018) remains a major =
concern in Afghanistan. This lower immunization coverage is one of the =
major contributing factors to the country's polio-endemic status. The =
backbone of the routine immunization program service delivery is the =
primary health care health facilities which in turn are responsible for =
catering to all health needs, including immunization. However, only =
facility-based immunization services cannot achieve good immunization =
coverage; therefore, a provision of outreach services has been made and =
needs to be implemented.

=20

With your commitment and efforts, immunization services have continued =
and sustained despite the COVID-19 pandemic and political transition. =
Now, the access to all areas has improved post-August 2021, but the =
coverage has not shown much improvement in the absence of robust =
outreach service planning and implementation. Therefore, strengthening =
outreach services is critical to use available opportunities and reach =
more children in all areas. This is integral to the overall =
responsibility of the health facility and implementing partner to =
improve coverage in its catchment area through a combination of =
facility-based and outreach service delivery mechanisms. But, =
understanding the gaps and the need to strengthen the program with the =
main focus on outreach immunization service delivery, NEPI has developed =
an accelerated vaccination approach with the provision of funding to =
support the plan. For this purpose, NEPI has identified 152 districts in =
30 provinces with a total of 990 Health Facilities through an assessment =
based on a combination of coverage and risk factors. The funding is =
being provided for a period of FOUR MONTHS to implement outreach =
services through the deployment of vaccinators (200 in total with =
distribution as per the list attached). The funding provision has been =
made to support salaries and their training and mobility. There is also =
a provision of funds to engage the existing network of CHWs. A detailed =
TOR, list of districts and budget sheet is attached.

The objective of this intervention is to achieve a minimum of 60% of =
Penta-3 coverage in all these districts or a minimum of 20% more than =
the current baseline (for example, if a district has a baseline of 50% =
Penta-3, then the target is minimum 70%). However, please use the =
opportunity to maximize the reach and coverage.

=20

A detailed TOR, list of districts and budget sheet is attached. The =
budget is planned for a total of four months but it will be aligned with =
the period of current contract and any continuation thereafter.=20

=20

All work, including planning, implementation and monitoring, has to be =
done in coordination with PEMTs/REMTs and UNICEF zonal offices. UNICEF =
zonal offices and Extenders will also support the process and monitor =
the implementation.

Ahmad, Immunization Specialist and Dr. Rajeev Gera (Immunization =
Manager) and they will also orient on this.

significant investments in the immunization program, lower routine =
immunization coverage ( ~60% for Penta3_AHS2018) remains a major concern =
in Afghanistan. This lower immunization coverage is one of the major =
contributing factors to the country's polio-endemic status. The backbone =
of the routine immunization program service delivery is the primary =
health care health facilities which in turn are responsible for catering =
to all health needs, including immunization. However, only =
facility-based immunization services cannot achieve good immunization =
coverage; therefore, a provision of outreach services has been made and =
needs to be implemented.<o:p></o:p><o:p>&nbsp;</o:p>With your =
commitment and efforts, immunization services have continued and =
sustained despite the COVID-19 pandemic and political transition. Now, =
the access to all areas has improved post-August 2021, but the coverage =
has not shown much improvement in the absence of robust outreach service =
planning and implementation. Therefore, strengthening outreach services =
is critical to use available opportunities and reach more children in =
all areas. This is integral to the overall responsibility of the health =
facility and implementing partner to improve coverage in its catchment =
area through a combination of facility-based and outreach service =
delivery mechanisms. But, understanding the gaps and the need to =
strengthen the program with the main focus on outreach immunization =
service delivery, NEPI has developed an accelerated vaccination approach =
with the provision of funding to support the plan. For this purpose, =
NEPI has identified 152 districts in 30 provinces with a total of 990 =
Health Facilities through an assessment based on a combination of =
coverage and risk factors. The funding is being provided for a period of =
FOUR MONTHS to implement outreach services through the deployment of =
vaccinators (200 in total with distribution as per the list attached). =
The funding provision has been made to support salaries and their =
training and mobility. There is also a provision of funds to engage the =
existing network of CHWs. A detailed TOR, list of districts and budget =
sheet is attached.<o:p></o:p><o:p>&nbsp;</o:p>The =
objective of this intervention is to achieve a minimum of 60% of Penta-3 =
coverage in all these districts or a minimum of 20% more than the =
current baseline (for example, if a district has a baseline of 50% =
Penta-3, then the target is minimum 70%). However, please use the =
opportunity to maximize the reach and coverage.<o:p></o:p><o:p>&nbsp;</o:p>A detailed =
TOR, list of districts and budget sheet is attached. The budget is =
planned for a total of four months but it will be aligned with the =
period of current contract and any continuation =
thereafter. <o:p></o:p><o:p>&nbsp;</o:p>All work, including planning, implementation and =
monitoring, has to be done in coordination with PEMTs/REMTs and UNICEF =
zonal offices. UNICEF zonal offices and Extenders will also support the =
process and monitor the implementation.<o:p></o:p><o:p>&nbsp;</o:p><o:p>&nbsp;</o:p>In UNICEF, =
