The technical proposal is revised where all the comments are addressed =
and the revised proposal is attached here for your further review and =
reference. If there is any additional information or clarification =
required please let us know.=20

The budget revision was very tough and we barely reduced the total =
estimated budget amount as per your proposed ceiling amount to minimum =
level. Please find the JACK justifications and comments for the changes =
and your comments under column J highlighted by yellow color. The =
previous submitted budget was according to hospital scope and =
requirements and it was estimated as per hospital management actual =
information reported in our need assessment report. There were very less =
chances to revise and reduce the budget total amount with significant =
decrease to cover the ceiling limit because it is very difficult to =
manage such a large hospital with less financial support while it is =
functional at the capital and serving patients around the country. The =
hospital is serving infection diseases that=E2=80=99s why health workers =
are receiving 100 percent risk allowances as per standard health =
guideline of MoPH. At other hand, due to recent political change and low =
economic condition of the Afghan citizens, the dependency on =
governmental hospitals are increasing day by day and it result the bed =
occupancy reaching above 100 percent per month therefore additional =
financial support is required to fulfill the medicines, lab regents, =
medical supplies and operational needs of the hospital. It is also worth =
to mention that it is one of the largest infection disease hospital in =
all over Afghanistan as explained in our technical proposal therefore =
any subsequent decrease in estimated budget will significantly affect =
the quality of health services in said hospital. The given ceiling =
amount is not following the WHO budget standard (2000 USD for each bed =
per month).

Therefore, please find our revised attached budget which is exceeding =
from amount 1.2m USD and accept above justification to approve the =
budget as estimated and reflected in attached program document. This =
will help us to extend the operational support with due efficiency and =
effectiveness to bring desirable change in the quality of the health =
service delivery.=20
