Dear  Respected JACK Colleagues

I have did supportive supervision from Bagh-e-Maidan CHC and Anamak BHC in
this two HF the Nutrition Service is very poor Hope you will follow up the
findings

1. Bagh-e-Maidan CHC
As I checked and it looks there is no person assigned for OPD SAM Service.
In Sawr 1401 there is no admission in OPD SAM. It is not acceptable
The case finding is very poor for OPD SAM in HF and HPs level.
There is no target for OPD SAM.
There is no space and room for OPD SAM.
The filing system for OPD SAM Treatment cart is very poor
Not doing appetite test for OPD SAM Children
The treatment cart is not filled properly.
Nutrition promotion is not a topic in health shura meetings in the Monthly
base.
The Drugs and Especially RUTF Stock not properly managed
The Defaulters is not following by responsible person and not listed

2. Anamak BHC
Not hired yet NC
There is no target for OPD SAM.
The filing system for the OPD SAM Treatment cart is very poor and not
available report and registration sheet for OPD SAM
Not doing an appetite test for OPD SAM Children
The treatment cart is not filled properly.
Nutrition promotion is not a topic in health shura meetings in the Monthly
base.
The Defaulters is not following by responsible person and not listed
Not Yet hired BHC Nurse

3. As well I have visited Orati BHC

The findings in Orati BHC RUTF is finished on 23 Hamal 1401 and very late
sent the supply on 31 Sawr 1401

Hope Nutrition Responsible Person  in JACK Office takes an action plan for
above Findings.
