DONE 2- Assessment /study final report along with specific plans related to availability and use of immunization service. An assessment report including the number of unvaccinated and or incomplete vaccinated children and CBAs along with mapping of area. NOT DONE 3- Very low coverages of all antigens, the organization must justify the low coverage.DONE 4- Budget balance sheet of Second quarter.NOT DONE 5- Male social mobilizer should be replaced with Female social mobilizer NOTDONE 6- The content of Supervision reports of the project is the same as the previous month. The update supervision report for the latest quarter is required.DONE 7- No supervision plan is attached with report DONE 8- Routine coordination meeting has no any signature DONE 9- No copies of the PHCC meeting, please include the hard copies with report.DONE 10- No separate sheet of FORM TECH-1 VACCINATION COVERAGE DATA SHEET with sign and signature of PPHD AND RMET.
