Dear Dr.Sahib Ghani,

Below finding is realted to BMZ weak point to consider please with team.

1.      On 16/11/2020 conducted meeting with Provincial staff and submitted 
our mission plan and objective.

a.      Conducted Monitoring and supportive supervision from BMZ project 
Coordinator activities,

-          Filling system were not arranged,

-          Project Coordinator JD was not existed

-          The copy monthly report were not existed in some of month  from 3 
MHTs in the office

-          Not existed monthly action of Project coordinator.

-          Project coordinator not have supportive supervision action plan.

-          No any tools (checklist) were existed with project coordinator 
to fill during supportive supervision

-          Not existed supportive supervision follow up action plan which 
found by Project coordinator.

b.      On 18/11/2020 conducted joint supportive supervision with Dr.Abdul 
Malik Wardak and Dr.Shafi

-          One day 64  patient  were registered but 73 prescription were 
existed

-          Antibiotic consumption were 58% and analgesic consumption were 
56%

-          Daily consumption record were not existed

-          Not conducted health education and No Health Education plan

-          Not triage of client for health services

Not Social destinations of the client were care for COVID-19 prevention
