Dear Dr. Sahib Noor Ahmad,

As per our phone call discussion yesterday. Kindly update us on the progress 
of the attached findings, and your plan for addressing the key findings 
(listed below) from our colleagues who recently conducted field visit from 
Kunduz  (19-26 August).

-       Frequent turnover of the project coordinator

-       Near to expire pharmaceutical list was not available to guide the 
team to prepare the distribution plan for the near to expiry items before 
its expiration. Two items were about to expire (end of September 2020) with 
very huge quantity: 1. 6,280 bottles of Paracetamol 120 mg syrup; 2. and 
6,000 bottles of Co- amoxiclav 156.25 mg syrup. Additionally, 40,100 zinc 
tablets will be expired on November 2020.

-       3,840 Charcoal tablets, and 20 Lidocaine ampules were available in 
pharmaceutical stock with zero average monthly consumption. It is most 
likely these items will remain in stock till end of the project with no use.

-       Since start of the project there were no evidence of physical 
inventory of pharmaceuticals and medical consumables.

-       The new enrollment of MAM and SAM children were limited that led to 
over stocking of RUSF (478 cartons) in the medical warehouse of JACK.

-       Pharmaceutical were miss placed in pharmaceuticals shelves almost 
3-meter-high form the floor. Syrups were placed at the top of shelves which 
have the risk of damage (fall down) if the shelves are shacked.

-       No rental vehicle for supportive supervision of BMZ supported mobile 
health teams by JACK project staff (Budget is allocated for this).

-       The project staff is not attending relevant provincial meetings to 
represent the project, and EPI report is not submitted to DoPH for six 
months.

-       National nutrition online database was not updated for SAM and MAM 
reports.

I hope to receive your update by tomorrow Thursday 3rd September before CoB.
