Dear Dr. Sahib Kazimi

I have done supportive supervision from Surat Khil BHC of Bagram district
and the following findings are shared for your kind follow up and as well
on the job training is provided for some findings which could be solved
during the visit.

There is no Filing system for  admission, cure and  defaulter please
provide the files.
There is no heater in delivery room it was really cold
The heater in NC Room is broken/ not properly active
The main problem is fire wood, the quality of wood is very poor/ Tar ast.
it is usually the same problem in most of the HFs which I have visited
There is no scale in MCH Room
The average use of RUTF is high based standart/ 90 sachet per child
There is no defaulter list for follow up
AWG and ALS is not calculated properly (On the job training is provided)
The appetite test is not conducting for SAM and MAM Children
There is no refer sheet/ card  in the HPs (as said by CHS)
The essential drug is not available in the HPs
For Hygiene issues the water system is not active in the HF

And Please share the two finding with EPI and Administration  in you office
The refrigerator is broken not active in EPI Section (it is said by
Vaccinator)
The desk and furnitures mostly broken in the HF
As well there is no electricity in the HF
