cleaner and guard for FATP trauma care project were absent. They were 
previously instructed if they take leave, they should take the signature of 
their replacer. However, unfortunately both of them were absent at same

At time of visit, FATP was not sterilized by chlorine, bathrooms were not 
cleaned and even dressing room was not sterilized.

Based on decision, both of them should receive written warning for such 
action.
