Greeting,

Today I had meeting with NEOC for submission of 2nd quarter 2021 GAVI 
Project report which they listed me below  documents and task which 
highlighted in yellow color and not done yet and we should be prepare and 
submit on Sunday 17/april/2021 therefore please prepare below documents 
ASAP.

1-    Original copy of Quarterly Technical Progress report should be by 
signing and stamping the PPHD and REMT with name and position and date. DONE

2-    Assessment /study final report along with specific plans related to 
availability and use of immunization service. An assessment report including 
the number of unvaccinated and or incomplete vaccinated children and CBAs 
along with mapping of area. NOT DONE

3-    Very low coverages of all antigens, the organization must justify the 
low coverage.DONE

4-      Budget balance sheet of Second quarter.NOT DONE

5-      Male social mobilizer should be replaced with Female social 
mobilizer NOTDONE

6-     The content of Supervision reports of the project is the same as the 
previous month. The update supervision report for the latest quarter is 
required.DONE

7-     No supervision plan is attached with report DONE

8-    Routine coordination meeting has no any signature DONE

9-    No copies of the PHCC meeting, please include the hard copies with 
report.DONE

10-                    No separate sheet of FORM TECH-1 VACCINATION COVERAGE 
DATA SHEET with sign and signature of PPHD AND RMET. DONE BUT SEND THD 
ORIGINAL ONE

11-                    In the part of narrative descriptions in TQR has 
mentioned that due to cold weather families didn’t let their child for 
vaccination hope you cover the missed child vaccination in the current 
quarter.it is as suggestion

12-                    Lack of transportation for female staff, this is the 
responsibility of NGO to solve the problem it is as suggestion

Thank you for submission of Second Technical Quarterly Report of Urban 
Immunization of (Kunduz)- Flex project. The assigned technical committee 
reviewed the report and the following issues need to be strictly considered. 
Therefore, you are requested to please take necessary action according to 
TOR and provide us with the missing documents and forms which are necessary 
during this period of the project. The deadline for submission of revised 
report and documents is 7th April, 2021.
