Dear COVID-19 Implementing SPs,

Since you were requested in the past as well to report the COVID-19 data 
through DHIS-2 on daily basis, still daily reporting of COVID-19 data by 
some SPs through DHIS-2 are not satisfactory.

Following are the provinces which either have no reported or reported low 
cases of COVID-19 data through DHIS-2

I.            Provinces which have not reported the contact tracing 
data through DHIS-2.

1.      Badakhshan    AKF

2.      Jawzjan           SAF

3.      Farah               MRCA

4.      Helmand         BRAC

5.      Paktika           OHPM

II.            Provinces which have reported the contact tracing low or 
zero (0):

1.      Baghlan           BDN

2.      Balkh               BDN

3.      Daikundi         MOVE

4.      Ghor                CHA

5.      Kabul              SDO

6.      Kunar              HN-TPO/ORCD

7.      Nanagarhar     AADA/HN-TPO

8.      Paktika            OHPM

9.      Paktia              HEWAD/NAC

10.  Urozgan          SHDP

III.            Provinces haven’t done contact tracing for last 14 days 
(mobile contact):

1.      Badakhshan    AKF

2.      Bamyan           AKF

3.      Ghor                CHA

4.      Herat               AADA

5.      Knadahar         BARAN/OHPM

6.      Kapisa             SCA

7.      Parwan            SCA

8.      Kunduz           JACK

9.      Samangan       AHEAD

10.  Takhar             BDN

In order to have an accurate and unique reporting system from all COVID-19 
wards/hospitals, RRTs and DCs through the DHIS-2, you are once again 
requested to report the COVID-19 data on daily basis. In addition, all 
COVID-19 SPs are requested to send/submit the contact tracing report (excel 
sheet) to the MoPH at the end of each month.

In case of need for support/cooperation on data qualification of contact 
tracing,  please contact with Ms. Momina Rahimi though phone (0700481984).

Dear COVID-19 Implementing SPs,

In reference to the second 6-month COVID-19 contract extension under the 
ERHSP project, daily reporting of COVID-19 data through DHIS-2 is one of the 
contractual obligations and SPs responsibility.

As observed, the overall daily reporting of the SPs are not satisfactory, in 
the meantime, the following provinces have not reported their COVID-19 data 
regularly:

1.      Samangan        AHEAD

2.      Farah               MRCA

3.      Bamyan           AKF

4.      Faryab             SDO

5.      Laghman         HN-TPO

6.      Paktia              HWAD/NAC

wards/hospitals, RRTs and DCs through the DHIS-2, you are once again 
requested to report the COVID-19 data on daily basis. Further noncompliance 
may affect and underestimate your performance.
