According to Supportive Supervision plan, on behalf of C team, I visited 
from Merbachakot DH, so I would you like to share the visit bold points with 
you for follow up:

1-      Visit achievement:

*  Analysis of indicators Performance with cause analysis of low performance 
(under five counselling and cure rate).

*  Conduct meetings with hospital board and indicators related staff to 
improve low achievement indicators.

*  Conduct meetings with the DC and RRT team and head of DH for more 
coordination and link DC, RRT with DH management and close coordination.

*  Selection of place for DC but need for some partition.

2-      Improvable Points:

BPHS:

*  Septic well of DH was full.

*  5 Oxygen slanders were in PO.

*  Stock out of drugs.

*  Two positions (anaesthesiology and male nurse) were vacant.

*  Water system needs repairing.

*  Low performance of U5C and TB cure rate.

*  Poor coordination between the head of DH and other staff.

COVID-19:

*  There was not any monthly target for project indicators.

*  Lab technician position was vacant.

*  Unavailable attendance sheet.

*  Stock out of medicine and other related items.

*  Temperature of the DC Room was so high, needed to change room (place for 
CD selected, but needed for some partitions, doors etc) or supply a fan.

That more weak point is discussed with DH staff, PO colleagues and will 
follow up, but other findings which need time for improvement, hope to 
improve on time.
