Dear Dr.Sahib Noor Ahmad,

Greeting from Kunduz,

According to our mission plan we completed successfully our mission  and 
listed  the summery of these activity  which have be done  as following:

1. Conducted meeting with provincial colleagues and submitted our mission 
plan
2. Conducted 14 HFs monitoring and provided each HFs follow up action for 
taking prompt action
3. Conducted meeting with provincial team and presented finding feedback of 
14 HFs with follow action plan with finding evidence (Picture of each 
finding)
4. Filled M&E checklist provincial office and HFs
5. Reviewed Progress of Project work plan
6. Monitoring of Malaria project
For more information we will have presentation regarding of our mission 
while we come back to Kabul  main office.

plan we have mission to Kunduz province which will be started our mission on 
04.Dec.2021. our mission objectives are monitoring of Sehatmandi Project, 
Malaria project and TSFP Project which are mentioned the details of our 
mission is attached. If you add any more task which we missed please let us 
know.

transport and accommodation for the mentioned mission.
