Based on our mission plan we completed successfully our mission from 4 Dec 
2021 up 11 Dec 2021 and  listed  the summery of these activity  which have 
be done  as following:

1.      Conducted meeting with provincial colleagues and submitted our 
mission plan

2.      Conducted 14 HFs monitoring and provided each HFs follow up action 
for taking prompt action

3.      Conducted meeting with provincial team and presented finding 
feedback of 14 HFs with follow action plan with finding evidence (Picture of 
each finding)

4.      Filled M&E checklist provincial office and HFs

5.      Reviewed Progress of Project work plan

6.      Monitoring of Malaria project

Therefore please kindly request to find the attached file of our mission 
plan report with follow up action plan and we expected to apply these 
findings to all 75 HFs for  coming changes to be achieve  high  performance 
and  excellent result of these findings and should be submitted the progress 
of each finding according to the deadline which set it in action plan 
accordingly.

Method of monitoring:

Based on our mission plan we had plan to conduct monitoring from 20 HFs, 
therefore we divided in two group which each group should be have monitoring 
10 HFs base on date just one group conducted 14 HFs.
