As you are in the picture on 16 Nov 2021 the evaluation committee conducted
meeting regarding end of project report of SRI and UI Projects and the
following missed documents are necessary to be provided for the assigned
committee *ASAP and this is worth mentioning that the time is very short
and your soon action is very necessary and important;*

*Inception report:*

*First quarter*:

*Second Quarter:*

-Copies of the monthly report of *month of Quas 1399*  give to BALKH REMT
must be attached and submitted alongside quarterly report.

- the date of reporting of quarterly technical progress report is not
corrected

* Third Quarter:*

- the date of reporting of quarterly technical progress report is not
corrected.

Quarterly financial Expenditure report and ledger (3 months activities
),Expenditure report, Tax payment, copy of salary pay sheet ledeger for the
reporting period, and Asset and equipment's inventory list.

The missing document should be showed to committee members

*Fourth quarter:*

Quarterly financial Expenditure report and ledger (3 months=E2=80=99 activi=
ties),
Expenditure report, Tax payment, copy of salary pay sheet ledeger for the
reporting period, and Asset and equipment's inventory list

*End of project Report:*

*In the report regarding Human resource looking some discrepancy  such as
page 5 =E2=80=93 12 diagnostic teams in page 11 =E2=80=93 20 diagnostic tea=

table of Human resource 24 members*

*And in page -6 has written refresher training though the training is
initial not refresher*

*Financial part of end project report*

1-   Invoice for payment (with 10 days after each reporting period (Finance=

2-   Asset and equipment=E2=80=99s inventory list (Finance)

3-   A-Z Ledger and expenditure report along with the end of project
report.(Finance).

*In the end of project report in the part of executive summary must be
written the main activities from start of the project up to end of project
only about one page.*
