Dear SDO,SAF,AADA,JACK Malaria focal points,

As I discussed through cell phone communication and based on bellow =
email the hand over and take over process of malaria grant will be taken =
on 27,Oct,2021 among SDO, SAF, AADA and JACK organization. Therefore =
kindly requesting all colleagues to consider the following points:

1.	JACK representative should come to SDO main office at 9:00 AM on =
27,Oct,2021 to take over of malaria grant of Kabul province from SDO.
2.	AADA representative will be present in SDO  Zabul Province office( =
Dr. Sahib Aub ) on 27,Oct,2021 to take over of malaria grant of Zabul =
province and Dr. Sahib Fahim  from SDO office sent all hand over =
documents through email to Zabul provincial team as I discussed with =
him.
3.	SAF representative will be present in SDO Faryab  Province office( =
Dr. Sahib Tareen or their BPHS team representative ) on 27,Oct,2021  at =
9:00 o=E2=80=99clock am  to take over of malaria grant of Faryab =
province from SDO and Dr. Sahib Fahim  from SDO office sent all hand =
over documents through email to Faryab provincial team as I discussed =
with him
4.	From UNDP program management unite my self and Mr.Shahdab will be =
available in SDO office at 9:00 o=E2=80=99clock on 27,Oct,2021. we will =
monitor the take over and hand over process and  also  take over all =
assets belong to CMS  from SDO.
5.	When the hand over and take over process completed in all provinces =
the scan copy will share through SDO colleagues with UNDP/GF.
6.	We strongly requesting from new SRs to hire current case management =
focal points of Kabul, Zabul and Faryab provinces, because they received =
several malaria related trainings and have several years field =
experience.

Attached please find SR agreement closure signed letter.

Based on my below email, hand over date is 31 Oct 2021, therefore we =
will follow up with both side (SDO and new SRs) to complete hand over =
process by 31 Oct.

To be in save side and have more time for quality end of project and =
FACE reports, UNDP considered 15th November as closure date for SR =
agreement.

Note: As mentioned above, hand over process should be completed by end =
of Oct and provincial staffs contract should be ended by mentioned date, =
only central office staffs (Project officer and finance officer) will =
continue up 15th Nov.

We will communicate with new SRs to hire current case management focal =
point, because they received several malaria related training and have =
several years field experience.

Based on my below email, Malaria amendment were singed with JACK, SAF =
and AADA for Kabul, Faryab and Zabul provinces, therefore we will be =
officially close UNDP malaria grant SR contract with SDO by the end of =
October 2021, the official template will be shared soon.

Please speed up the hand over process of malaria grant to the mentioned =
new BPHS implementer NGOs. Please also insure the submission of project =
end report and final FACE report before 15th of November 2021.

Dear Dr. Sahib Hedayat

As you better know, the SRs selection under malaria grant was =
programmatic (BPHS implementer NGOs were selected in each provinces).  =
Based on the Government=E2=80=99s changes to the BPHS implementers, SDO =
will hand over the BPHS in Kabul, Faryab and Zabul provinces to JACK, =
SAF and AADA, therefore the Malaria grant SR agreement will be =
terminated after the signature of the new BPHS contracts with mentioned =
NGOs.
