Dear JACK Colleagues,

Thank you for submitting the Fourth Technical Quarterly Report of UI of =
(Kunduz)- Flex project. The assigned technical committee reviewed the =
report and the following issues need to be strictly considered. =
Therefore, you are requested to please take necessary action according =
to TOR and provide us the missing documents and forms which are =
necessary during this period of the project. The deadline for submission =
of revised reports and documents is 28th  of Oct, 2021.

- Any minute of coordination meeting with Mula and haj and Aohqaf other =
people regarding allowing team to be vaccinated children in the mosque=20

- In the part of the narrative what does it mean of this paragraph =
During the reporting period, all staff were present on their duties =
their activities were in accordance with the Project policy and staff =
the project was completed with no vacancies.

- The hard copy of the reports must be approved by the -  provincial =
team of the directorate of public health and REMT. Please submit the =
report with approval of relevant authorities.=20

- The project work plan progression is not according to TOR and must be =
prepared accordingly.=20

- No update list of staff (hard and soft).

Reason behind low coverage of all antigens what necessary action have =
you taken for improvement and high drop out of pant 3.

Regarding the weak point of supervision, what necessary measurements you =
have taken and please provide us with any document or any action plan =
for improvement of the project.=20

A Balanced budget sheet should be included in the report.

Verification of the community is part of the report.=20
