Dear All!

According to Supportive Supervision plan, on behalf of *C team*, I visited
from Merbachakot DH, so I would you like to share the visit bold points
with you for follow up:

*1-      **Visit achievement:*

=C3=98  Analysis of indicators Performance with cause analysis of low
performance (under five counselling and cure rate).

=C3=98  Conduct meetings with hospital board and indicators related staff t=
o
improve low achievement indicators.

=C3=98  Conduct meetings with the DC and RRT team and head of DH for more
coordination and link DC, RRT with DH management and close coordination.

=C3=98  Selection of place for DC but need for some partition.

*2-      **Improvable Points:*

*BPHS:*

=C3=98  Septic well of DH was full.

=C3=98  5 Oxygen slanders were in PO.

=C3=98  Stock out of drugs.

=C3=98  Two positions (anaesthesiology and male nurse) were vacant.

=C3=98  Water system needs repairing.

=C3=98  Low performance of U5C and TB cure rate.

=C3=98  Poor coordination between the head of DH and other staff.

*COVID-19:*

=C3=98  There was not any monthly target for project indicators.

=C3=98  Lab technician position was vacant.

=C3=98  Unavailable attendance sheet.

=C3=98  Stock out of medicine and other related items.

=C3=98  Temperature of the DC Room was so high, needed to change room (plac=
e for
CD selected, but needed for some partitions, doors etc) or supply a fan.
That more weak point is discussed with DH staff, PO colleagues and will
follow up, but other findings which need time for improvement, hope to
improve on time.
