SITUATION OVERVIEW
MoPH Figures: As of 28 July=2C MoPH data shows that 145=2C996 people acros=
s all 34 provinces in Afghanistan are confirmed to have had COVID-19 since=
 the start of the pandemic. An additional 8=2C143 people have tested posit=
ive since the last update two weeks ago. Of the total cases since the star=
t of the pandemic=2C some 95=2C726 people have recovered=2C and 6=2C615 pe=
ople have died =E2=80=93 at least 93 of the fatalities have been healthcar=
e workers. Since the start of the pandemic=2C only 699=2C172 tests have be=
en conducted for a population of 40.4 million.

Cases peaked over the post-Eid al-Fitr period and have now started to rece=
de. In the last week=2C some 2=2C812 new cases and 290 new deaths were rep=
orted. This is a 47 per cent decrease in the number of new cases and a 15=
 per cent decrease in deaths compared to the week prior. While there are i=
ndications that the third wave is receding=2C caution is advised as post-E=
id al-Hada trends are monitored.

Overseas testing has confirmed the presence of the variants in Afghanistan=
=2E While Afghanistan lacks in-country facilities to test for the Delta vari=
ant=2C concern over the variant=E2=80=99s spread remains high. A total of=
 35 laboratories are now operating with testing capacity of 8=2C700 tests=
 per day. There are plans to establish 9 more labs by the end of August. D=
ue to limited public health resources=2C lack of people coming forward for=
 testing=2C as well as the absence of a national death register=2C confirm=
ed cases of and deaths from COVID-19 are likely to be under-reported overa=
ll in Afghanistan.

Health partners continue to support the health system in case management a=
nd infection prevention and control. Since the onset of the pandemic in th=
e country=2C around 7=2C434 health professionals have been trained in resp=
onse to COVID-19 at national and provincial level on subjects such as crit=
ical and severe case management=2C using of BIPAP=2C CiPAP=2C ventilators=
=2C nursing care=2C anesthesia=2C intubation and infection prevention and=
 control. WHO has also recently delivered 500 oxygen concentrators to the=
 Ministry of Public Health for distribution to COVID hospitals across the=
 country.

WHO warns that despite the evidence of the Delta variant and ongoing risks=
 of future variants=2C widespread complacency and failure to follow public=
 health advice in Afghanistan is creating grave risks in the community wit=
h people generally not observing physical distancing or mask-wearing proto=
cols. Given the decreasing yet still high number of in cases=2C the Govern=
ment of Afghanistan has extended closure of all schools=2C universities an=
d training courses that started on 29 May. The school closure is in effect=
 across 16 provinces=2C including Kabul. Most government offices have move=
d to essential staff only because of the outbreak. No nation-wide lockdown=
 measures are currently in place as a result of the pandemic.

Vaccination: Since 8 March=2C Afghanistan has received close to 5 million=
 doses of COVID-19 vaccines =E2=80=93 468=2C000 from the COVAX facility=2C=
 500=2C000 directly from the Government of India=2C 700=2C000 from the gov=
ernment of the People's Republic of China and 3.3 million from the USA. An=
 additional 600=2C000 doses of vaccine are also expected to arrive to the=
 country in the coming month.

COVID-19 vaccinations delivered through MoPH are open to all those above 1=
8 years of age. Vaccination is currently available in select health facili=
ties and through mobile vaccination teams. People are reminded that mixing=
 of vaccines between doses is not recommended.

More than 1.277 million people have been vaccinated against COVID-19 in Af=
ghanistan as of 28 July 2021. Of the total people vaccinated=2C almost 61=
 per cent are men and 39 per cent are women=2C almost 146=2C00 (11 per cen=
t) are health care workers=2C more than 128=2C000 (10 per cent) are teache=
rs=2C and over 8=2C000 (1 per cent) are returnees from neighboring countri=
es.

Overall=2C there remains strong concern regarding equitable access to vacc=
ines for Afghans=2C especially vulnerable groups such as IDPs=2C returnees=
 and nomadic populations and people living in hard-to-reach areas. IOM con=
tinues to report that vaccination coverage is extremely limited among all=
 migrant populations due to negative perceptions and barriers to accessing=
 care. Greater focus is needed to ensure mobile populations are vaccinated=
 on pace with settled populations given the impact mobility has as a vecto=
r for transmission=2C especially with the more contagious viral variants p=
resent in neighbouring countries. Additional efforts are also needed to re=
ach women and people living in non-government-controlled areas with vaccin=
es. Risk communication and community engagement efforts are needed to ensu=
re high-risk populations and frontline staff understand the benefits of th=
e vaccine and can effectively dispel misinformation. To this effect=2C the=
 RCCE Sub-Working Group worked in collaboration with
the Ministry of Public Health to develop and disseminate COVID vaccination=
 discussion guides. The guides reflect concerns and questions about the va=
ccine that have been recently expressed.

Health partners are very concerned about the exponential increase in hosti=
lities across the country and the impact this is having on people=E2=80=99=
s ability to access care. Health Cluster partners recorded a total of 30 i=
ncidents in 18 districts across 8 provinces impacting health facilities an=
d personnel in the first six months of 2021 in which 13 health facilities=
 were affected=2C 29 health care workers were severely affected (12 killed=
=2C 10 injured 7 detained)=2C and 6 patients were severely affected (1 inj=
ured=2C 5 killed). As health facilities and resources are increasingly str=
etched due to both COVID and escalating conflict=2C all parties to the con=
flict are reminded of their obligations to respect and protect health care=
 under IHL.

Socio-economic impacts: In addition to pre-existing challenges=2C the soci=
o-economic impacts of COVID-19 and drought have translated into a food ins=
ecurity crisis. The most recent IPC analysis estimates that 12.2 million p=
eople =E2=80=93 almost one third of the population - are in crisis or emer=
gency levels of food insecurity. Food prices are already higher than norma=
l due to COVID-19 and may increase further in some places due to conflict=
=2C disruptions at the border=2C and water scarcity.  Water scarcity is be=
ing seen in a number of areas and a significantly reduced wheat harvest is=
 expected.

Cross Border:
UPDATED
Border Crossings: Since 29 April=2C the Iranian Government has imposed an=
 indefinite lockdown with border closures. The border remains open only to=
 commercial traffic and movement of documented nationals returning home to=
 Afghanistan. According to the Border Monitoring Team of the Directorate o=
f Refugees and Repatriation (DoRR)=2C a total of 30=2C927 Afghanistan nati=
onals returned from Iran through the Milak and Islam Qala border crossing=
 sites between 9 and 22 July. Of these=2C more than 13=2C000 people return=
ed voluntarily and almost 18=2C000 were deported. Since the start of the y=
ear=2C a historically significant influx of more than 652=2C000 people ret=
urned to Afghanistan from Iran sparking new concerns about transmission of=
 COVID-19 variants and overall absorption capacity of existing health reso=
urces. A sharp drop in remittances to Afghanistan has also been observed.

Between 9 and 22 July 2021=2C 69 undocumented Afghans voluntarily returned=
 from Pakistan through the Torkham and Spin Boldak borders. Total returns=
 from Pakistan in 2021 remain low with fewer than 7=2C000 undocumented and=
 refugee returns.

The rapid escalation of conflict across Afghanistan has also had a pronoun=
ced impact on border crossing points. Humanitarian partners=E2=80=99 opera=
tions at the Spin Boldak and Islam Qala borders have been impacted by rece=
nt fighting and are temporarily suspended. IOM is exploring a resumption o=
f services as early as possible to ensure continued delivery of humanitari=
an assistance to returning Afghans. Similarly=2C the recent escalation of=
 conflict in Spin Boldak crossing point has affected humanitarian partners=
=E2=80=99 operations at the border.
