Dear Zalmay Ahmadzai

Following are the basis for calculating 20% Salary for Health facility
staff.

1.       Each health facility performance is measured against BPHS 11
indicators; these targets were agreed and assigned with health facility
staff at the start of year. Performance ratio of each HFs is calculated as
follows.

Performance ratio= Annual achieved targets multiplied by unit cost /divided
by annual assigned targets by unit cost

(If individual indicator performance of a single HFs is 120%, this has been
capped to 100%). If the rest of indicators performance is 100% or less than
100%. Actual achieved target is taken for the calculating performance of
HFs).

2.       Payment ratio= Performance ratio is multiplied with third party
monitoring ratio conducted by MOPH assigned staff e.g.  (If performance
ratio of health facility is 90% and TPM ratio is 100%= Payment ratio will
be 90%). 80% salary has already been paid to HFs staff, remaining 10%
salary will be will be paid if payment ratio will be 90%.

3.       If payment % ratio is above than 80%, then JACK will have to be
paid to health facility staff. In case if payment %age is less than 80%
then there will be no liability on the part of JACK.
