SITUATION OVERVIEW

MoPH Figures: As of 14 July=2C MoPH data shows that 136=2C643 people acros=
s all 34 provinces in Afghanistan are confirmed to have had COVID-19 since=
 the start of the pandemic. An additional 16=2C427 people have tested posi=
tive since the last update two weeks ago. Of the total cases since the sta=
rt of the pandemic=2C some 83=2C849 people have recovered=2C and 5=2C923 p=
eople have died =E2=80=93 at least 93 of the fatalities have been healthca=
re workers. Since the start of the pandemic=2C only 664=2C045 tests have b=
een conducted for a population of 40.4 million.

The number of cases remain at alarming levels over the post-Eid al-Fitr pe=
riod. The daily average number of new cases now far surpasses figures seen=
 during the peak of the first and second waves last year. In week the last=
 week=2C some 8=2C674 new cases and 508 new deaths were reported. This is=
 a 15 per cent decrease in the number of cases and 8 per cent decrease in=
 deaths compared to week prior.

Overseas testing has confirmed the presence of the variants in Afghanistan=
=2E While Afghanistan lacks in-country facilities to test for the Delta vari=
ant=2C concern over the variant=E2=80=99s spread remains high. Afghanistan=
 now has a daily test-positivity-rate =E2=80=93 positive tests as a percen=
tage of total tests =E2=80=93 of 48 per cent as of 14 July=2C suggesting o=
verall under-testing of potential cases. With a testing capacity of 8=2C70=
0 tests per day=2C 35 laboratories are now operating in and there are plan=
s to establish 9 more by the end of August. Due to limited public health r=
esources=2C lack of people coming forward for testing=2C as well as the ab=
sence of a national death register=2C confirmed cases of and deaths from C=
OVID-19 are likely to be under-reported overall in Afghanistan.

Health partners continue to support the health system in case management a=
nd infection prevention and control. Since the outset of the pandemic in t=
he country=2C around 7=2C434 health professionals have been trained in res=
ponse to COVID-19 at national and provincial level on subjects such as cri=
tical and severe case management=2C using of BIPAP=2C CiPAP=2C ventilator=
=2C nursing care=2C anesthesia=2C intubation and infection prevention and=
 control.

WHO warns that despite the new surge=2C widespread complacency and failure=
 to follow public health advice in Afghanistan is creating grave risks in=
 the community with people generally not observing physical distancing or=
 mask-wearing protocols. There is particular concern about the upcoming Ei=
d al-Adha period and the need to promote personal protection measures duri=
ng this holiday. Health partners have worked with MoPH to develop specific=
 messages for the upcoming Eid holidays. These will be widely circulated.=
 Given the rapid rise in cases=2C the Government of Afghanistan has extend=
ed closure of all schools=2C universities and training courses that starte=
d on 29 May. The school closure will be in effect across 16 provinces=2C i=
ncluding Kabul. Most government offices have moved to essential staff beca=
use of the outbreak. The Government has also announced other preventative=
 measures across these 16 provinces=2C including the banning of mass gathe=
rings and weddings. No other nation-wide lockdown measures are currently i=
n place.

Vaccination: Since 8 March=2C Afghanistan has received 3=2C068=2C000 doses=
 of COVID-19 vaccines =E2=80=93 468=2C000 from the COVAX facility=2C 500=
=2C000 directly from the Government of India=2C 700=2C000 from the governm=
ent of the People's Republic of China and 1=2C400=2C000 from USA. Some 2=
=2C192=2C000 additional doses of vaccines are expected to arrive to the co=
untry this month as well.

COVID-19 vaccinations through MoPH are open to all those above 18 years of=
 age. Vaccination is currently available in select health facilities and t=
hrough mobile vaccination teams. People are reminded that mixing of vaccin=
es between doses is not recommended.

Some 989=2C015 individuals have been vaccinated against COVID-19 as of 14=
 July 2021. Of this=2C 753=2C277 individuals have received the first dose=
 of the vaccine=2C and 235=2C738 have received the second dose. Of the tot=
al people vaccinated=2C 62.2 per cent are males and 37.8 per cent are fema=
les=2C 138=2C101 (14.2 per cent) are health care workers=2C 95=2C682 (9.7=
 per cent) are teachers=2C 14=2C231 (1.4 per cent) are those living in IDP=
 camps=2C and 7=2C081 (0.7 per cent) are returnees from neighboring countr=
ies.

According to a recent public perception survey on COVID-19 vaccines in Afg=
hanistan conducted by the Afghanistan National Public Health Institute (AN=
PHI) and UNICEF=2C 71 per cent of respondents reported that they are yet t=
o receive the COVID-19 vaccine. However=2C some 69 per cent of all respond=
ents noted that they wish to be vaccinated if given the opportunity=2C com=
pared to just 17 per cent who said they do not want to be vaccinated. Arou=
nd 14 per cent did not respond to the question. The majority of all respon=
dents listed the radio=2C masjids and religious leaders=2C TV and health c=
are providers as the most trusted means of communication for COVID-19 info=
rmation.

Overall=2C there remains strong concern regarding equitable access to vacc=
ines for Afghans=2C especially vulnerable groups such as IDPs=2C returnees=
 and nomadic populations and people living in hard-to-reach areas. IOM con=
tinues to report that vaccination coverage is extremely limited among all=
 migrant populations due to negative perceptions and barriers to accessing=
 care. Greater focus is needed to ensure mobile populations are vaccinated=
 on pace with settled populations given the impact mobility has as a vecto=
r for transmission=2C especially with the more contagious viral variants p=
resent in neighbouring countries. Additional efforts are also needed to re=
ach women and people living in non-government-controlled areas with vaccin=
es. Risk communication and community engagement efforts are needed to ensu=
re high-risk populations and frontline staff understand the benefits of th=
e vaccine and can effectively dispel misinformation. Countering negative r=
umours about the vaccine is a priority as well as
promotion of preventative measures during the upcoming Eid holiday.

Health partners are very concerned about the exponential increase in hosti=
lities across the country. Health Cluster partners have recorded a total o=
f 30 incidents in 18 districts of 8 provinces impacting health facilities=
 and personnel in the first six months of 2021 in which 13 health faciliti=
es were affected=2C 29 health care workers were severely affected (12 kill=
ed=2C 10 injured 7 detained)=2C and 6 patients were severely affected (1 i=
njured=2C 5 killed). As health facilities and resources are increasingly s=
tretched due to both COVID and escalating conflict=2C all parties to the c=
onflict are reminded of their obligations to respect and protect health ca=
re under IHL.

Socio-economic impacts: In addition to pre-existing challenges=2C the soci=
o-economic impacts of COVID-19 and drought have translated into a food ins=
ecurity crisis. The recently released IPC analysis estimates that 12.2 mil=
lion people =E2=80=93 almost one third of the population - are in crisis o=
r emergency levels of food insecurity. Food prices are already higher than=
 normal due to COVID-19 may increase further in some places due to conflic=
t=2C disruptions at the border=2C and water scarcity.  Water scarcity is a=
lready being seen in a number of areas and a significantly reduced wheat h=
arvest is expected.

The Afghan Government=2C UN and humanitarian partners held a joint appeal=
 on 11 July to highlight the worsening humanitarian situation due to COVID=
=2C conflict=2C and the drought. The appeal highlighted the urgency of the=
 humanitarian situation=2C the current gap in funding for the HRP and the=
 Government=E2=80=99s plan to resource existing National Priority Programm=
es in response to the drought.

CROSS-BORDER

Border Crossings: Since 29 April=2C the Iranian Government has imposed an=
 indefinite lockdown with border closures. The border remains open only to=
 commercial traffic and movement of documented nationals returning home to=
 Afghanistan. According to the Border Monitoring Team of the Directorate o=
f Refugees and Repatriation (DoRR)=2C a total of 16=2C957 Afghanistan nati=
onals returned from Iran through the Milak and Islam Qala border crossing=
 sites between 2^nd and 8^th July. Of these=2C 7=2C103 people returned vol=
untarily and 9=2C854 were deported. Since the start of the year=2C a histo=
rically significant influx of 621=2C133 people returned to Afghanistan fro=
m Iran sparking new concerns about transmission of COVID-19 variants and o=
verall absorption capacity of existing health resources. A sharp drop in r=
emittances to Afghanistan has also been observed.

Between 2^nd and 8^th July 2021=2C a total of 22 undocumented Afghans volu=
ntarily returned from Pakistan through the Torkham and Spin Boldak borders=
=2E The total humanitarian returns from Pakistan in 2021 remains low with 6=
=2C846 total undocumented and refugee returns.

The rapid escalation of conflict across Afghanistan has also had a pronoun=
ced impact on border crossing points. Humanitarian partners=E2=80=99 opera=
tions at the Spin Boldak and Islam Qala borders have been impacted by rece=
nt fighting and are temporarily suspended. IOM is exploring resumption as=
 early as possible to ensure continued delivery of humanitarian assistance=
 to returning Afghans. Similarly=2C the recent escalation of conflict in S=
pin Boldak crossing point has affected humanitarian partners=E2=80=99 oper=
ations at the border.

The number of cases remain at alarming levels=
 over the post-Eid al-Fitr period. The daily average number of new cases n=
ow far surpasses figures seen during the peak of the first and second wave=
s last year. In week the last week=2C some 8=2C674 new cases and 508 new d=
eaths were reported. This is a 15 per cent decrease in the number of cases=
 and 8 per cent decrease in deaths compared to week prior.<br>
<br>
Overseas testing has confirmed the presence of the variants in Afghanistan=
=2E While Afghanistan lacks in-country facilities to test for the Delta vari=
ant=2C concern over the variant=E2=80=99s spread remains high. Afghanistan=
 now has a daily test-positivity-rate =E2=80=93 positive tests as a percen=
tage of total tests =E2=80=93 of 48 per cent as of 14 July=2C suggesting o=
verall under-testing of potential cases. With a testing capacity of 8=2C70=
0 tests per day=2C 35 laboratories are now operating in and there are plan=
s to establish 9 more by the end of August. Due to limited public health r=
esources=2C lack of people coming forward for testing=2C as well as the ab=
sence of a national death register=2C confirmed cases of and deaths from C=
OVID-19 are likely to be under-reported overall in Afghanistan.<br>
<br>
Health partners continue to support the health system in case management a=
nd infection prevention and control. Since the outset of the pandemic in t=
he country=2C around 7=2C434 health professionals have been trained in res=
ponse to COVID-19 at national and provincial level on subjects such as cri=
tical and severe case management=2C using of BIPAP=2C CiPAP=2C ventilator=
=2C nursing care=2C anesthesia=2C intubation and infection prevention and=
 control.<br>
<br>
WHO warns that despite the new surge=2C widespread complacency and failure=
 to follow public health advice in Afghanistan is creating grave risks in=
 the community with people generally not observing physical distancing or=
 mask-wearing protocols. There is particular concern about the upcoming Ei=
d al-Adha period and the need to promote personal protection measures duri=
ng this holiday. Health partners have worked with MoPH to develop specific=
 messages for the upcoming Eid holidays. These will be widely circulated.=
 Given the rapid rise in cases=2C the Government of Afghanistan has extend=
ed closure of all schools=2C universities and training courses that starte=
d on 29 May. The school closure will be in effect across 16 provinces=2C i=
ncluding Kabul. Most government offices have moved to essential staff beca=
use of the outbreak. The Government has also announced other preventative=
 measures across these 16 provinces=2C including the banning of mass gathe=
rings and weddings. No other nation-wide lockdown measures are currently i=
n place.<br>
<br>
<strong>Vaccination:</strong> Since 8 March=2C Afghanistan has received 3=
=2C068=2C000 doses of COVID-19 vaccines =E2=80=93 468=2C000 from the COVAX=
 facility=2C 500=2C000 directly from the Government of India=2C 700=2C000=
 from the government of the People's Republic of China and 1=2C400=2C000 f=
rom USA. Some 2=2C192=2C000 additional doses of vaccines are expected to a=
rrive to the country this month as well.<br>
<br>
COVID-19 vaccinations through MoPH are open to all those above 18 years of=
 age. Vaccination is currently available in select health facilities and t=
hrough mobile vaccination teams. People are reminded that mixing of vaccin=
es between doses is not recommended.<br>
<br>
Some 989=2C015 individuals have been vaccinated against COVID-19 as of 14=
 July 2021. Of this=2C 753=2C277 individuals have received the first dose=
 of the vaccine=2C and 235=2C738 have received the second dose. Of the tot=
al people vaccinated=2C 62.2 per cent are males and 37.8 per cent are fema=
les=2C 138=2C101 (14.2 per cent) are health care workers=2C 95=2C682 (9.7=
 per cent) are teachers=2C 14=2C231 (1.4 per cent) are those living in IDP=
 camps=2C and 7=2C081 (0.7 per cent) are returnees from neighboring countr=
ies.<br>
<br>
According to a recent public perception survey on COVID-19 vaccines in Afg=
hanistan conducted by the Afghanistan National Public Health Institute (AN=
PHI) and UNICEF=2C 71 per cent of respondents reported that they are yet t=
o receive the COVID-19 vaccine. However=2C some 69 per cent of all respond=
ents noted that they wish to be vaccinated if given the opportunity=2C com=
pared to just 17 per cent who said they do not want to be vaccinated. Arou=
nd 14 per cent did not respond to the question. The majority of all respon=
dents listed the radio=2C masjids and religious leaders=2C TV and health c=
are providers as the most trusted means of communication for COVID-19 info=
rmation.<br>
<br>
Overall=2C there remains strong concern regarding equitable access to vacc=
ines for Afghans=2C especially vulnerable groups such as IDPs=2C returnees=
 and nomadic populations and people living in hard-to-reach areas. IOM con=
tinues to report that vaccination coverage is extremely limited among all=
 migrant populations due to negative perceptions and barriers to accessing=
 care. Greater focus is needed to ensure mobile populations are vaccinated=
 on pace with settled populations given the impact mobility has as a vecto=
r for transmission=2C especially with the more contagious viral variants p=
resent in neighbouring countries. Additional efforts are also needed to re=
ach women and people living in non-government-controlled areas with vaccin=
es. Risk communication and community engagement efforts are needed to ensu=
re high-risk populations and frontline staff understand the benefits of th=
e vaccine and can effectively dispel misinformation. Countering negative r=
umours about the vaccine is a priority as well as promotion of preventativ=
e measures during the upcoming Eid holiday.<br>
<br>
Health partners are very concerned about the exponential increase in hosti=
lities across the country. Health Cluster partners have recorded a total o=
f 30 incidents in 18 districts of 8 provinces impacting health facilities=
 and personnel in the first six months of 2021 in which 13 health faciliti=
es were affected=2C 29 health care workers were severely affected (12 kill=
ed=2C 10 injured 7 detained)=2C and 6 patients were severely affected (1 i=
njured=2C 5 killed). As health facilities and resources are increasingly s=
tretched due to both COVID and escalating conflict=2C all parties to the c=
onflict are reminded of their obligations to respect and protect health ca=
re under IHL.<br>
<br>
<strong>Socio-economic impacts:</strong> In addition to pre-existing chall=
enges=2C the socio-economic impacts of COVID-19 and drought have translate=
d into a food insecurity crisis. The recently released IPC analysis estima=
tes that 12.2 million people =E2=80=93 almost one third of the population=
 - are in crisis or emergency levels of food insecurity. Food prices are a=
lready higher than normal due to COVID-19 may increase further in some pla=
ces due to conflict=2C disruptions at the border=2C and water scarcity. &n=
bsp;Water scarcity is already being seen in a number of areas and a signif=
icantly reduced wheat harvest is expected.<br>
<br>
The Afghan Government=2C UN and humanitarian partners held a joint appeal=
 on 11 July to highlight the worsening humanitarian situation due to COVID=
=2C conflict=2C and the drought. The appeal highlighted the urgency of the=
 humanitarian situation=2C the current gap in funding for the HRP and the=
 Government=E2=80=99s plan to resource existing National Priority Programm=
es in response to the drought.<br>

<strong>Border Crossings: </strong>Since 29 Ap=
ril=2C the Iranian Government has imposed an indefinite lockdown with bord=
er closures. The border remains open only to commercial traffic and moveme=
nt of documented nationals returning home to Afghanistan. According to the=
 Border Monitoring Team of the Directorate of Refugees and Repatriation (D=
oRR)=2C a total of 16=2C957&nbsp;Afghanistan nationals returned from Iran=
 through the Milak and Islam Qala border crossing sites between 2<sup>nd</=
sup> and 8<sup>th</sup> July. Of these=2C 7=2C103&nbsp;people returned vol=
untarily and 9=2C854&nbsp;were deported. Since the start of the year=2C a=
 historically significant influx of 621=2C133 people returned to Afghanist=
an from Iran sparking new concerns about transmission of COVID-19 variants=
 and overall absorption capacity of existing health resources. A sharp dro=
p in remittances to Afghanistan has also been observed.<br>
&nbsp;<br>
Between 2<sup>nd</sup> and 8<sup>th</sup> July 2021=2C a total of 22 undoc=
umented Afghans voluntarily returned from Pakistan through the Torkham and=
 Spin Boldak borders. The total humanitarian returns from Pakistan in 2021=
 remains low with 6=2C846 total undocumented and refugee returns.<br>
&nbsp;<br>
The rapid escalation of conflict across Afghanistan has also had a pronoun=
ced impact on border crossing points. Humanitarian partners=E2=80=99 opera=
tions at the Spin Boldak and Islam Qala borders have been impacted by rece=
nt fighting and are temporarily suspended. IOM is exploring resumption as=
 early as possible to ensure continued delivery of humanitarian assistance=
 to returning Afghans. Similarly=2C the recent escalation of conflict in S=
pin Boldak crossing point has affected humanitarian partners=E2=80=99 oper=
ations at the border.<br>
