I shared the issues with the relevant RRTs and the sample  which has =
taken by Char Asyab RRT on the date of 8th July. But the contact tracing =
was  168 on the same date this is because every patient should be =
followed for three times: first date ,second date ,and third date most =
of them were from the previous week who completed the final date of =
their contact tracing  and the RRT compiled both on the same date and =
reported .

The remaining issues I gave feedback and it will be strictly followed =
with the relevant teams by their supervisors .

1-     RRT and DC supplies: as I talked with site supervisors, our RRT =
and DC teams needs supplies and our respected operation team should =
provide them in the earliest time. Those supplies are

a)     Mask

b)     Gloves

c)      Senitizer

d)     2 infrared thermometer

e)     Template print outs for RRT and DC

f)      Credit card for teams

g)      12 unit pulse oximeter

h)     VTM (the request should be followed up with MoPH)

i)       RTD (rapid test)

2-     Data entry and contact tracing updates and challenges: from first =
of July 2021, we have received three times the result of CPHL on 4th =
July, 6th July and 7th July 2021. We have entered all positive cases of =
4th and 6th July 2021, however, we were unable to enter the data of 220 =
positive cases from 7th July 2021. The reason was that the DHIS2 is not =
working properly. I am in contact with DHIS2 responsible of MoPH and we =
will solve the issue today 10th July 2021.

The contact tracing is ongoing by RRTs and DCs, however there are some =
challenges I would like to share and it is site supervisors duty to =
follow them up and report us back:

a)      In one RRT, there were only 2 positive cases however there were =
168 traces (RRT charasyab)

b)     RRTs and DCs don=E2=80=99t share ID of death reported cases

c)      One RRT does not share suspect, however they have taken many =
samples (VIP team)

d)     Suspect cases do not match samples. For example, according to our =
indicator, we have to take sample of each suspect, however, in weekly =
report I found out that there were (for example) 1000 suspect and 200 =
samples. We know that we are facing VTM shortage challenge, however, our =
report should be quality. If we take sample of 20, then we should report =
20 suspect. Other than this is not acceptable.

Above issues are related to Dr Negina Ahmadi and Dr Ziaulhaq Ahmadi, =
please follow up above issues and make sure the data we receive from =
field are quality data.

3-     Contact tracing (private sector and CPHL) update and challenges:

a)      On 4th of July 2021, 36 cases were reported to contact tracing =
team, they managed to trace 14 of them and found 22 contacts. However, =
22 of them were not traced due to multiple reasons (will share at the =
end).

b)     On 5th of July 2021, 18 cases were reported to contact tracing =
team, they managed to trace 18 of them and found 55 contacts. However, 7 =
of them were not traced due to multiple reasons.

c)      On 6th of July 2021, 19 cases were reported to contact tracing =
team, they managed to trace 13 of them and found 40 contacts. However, 6 =
of them were not traced due to multiple reasons.

d)     On 7th of July 2021, 14 cases were reported to contact tracing =
team, they managed to trace 17 of them and found 3 contacts. However, 7 =
of them were not traced due to multiple reasons.

e)     On 8th of July 2021, 15 cases were reported to contact tracing =
team, they managed to trace 10 of them and found 46 contacts. However, 5 =
of them were not traced due to multiple reasons.

f)       Today 10th July 2021, they received 38 cases and will report at =
the end of today.

number     3- No consent          4- switched off phone numbers
