Subject: Open Letter of Serious Concern on Sehatmandi Project =
implementation in Afghanistan

Dear Sir/Madam

NGOs played a vital role, for revitalization and sustaining an effective =
health system in Afghanistan during the last four decades and especially =
during the last twenty years. The contribution of NGOs, and closely =
working with MoPH and the trust developed by donors and community, =
enabled the health system to be a model approach in the country. This =
needs to be further built on, moved to the next level, and the =
opportunities to be used for the best of the country. The current =
Sehatmandi project was a new model, which is piloted in large scale, =
with positive results and sure with challenges. NGOs feel that this new =
modality of services delivery that is based on the principles of =
performance-based payment in the entire country, is a new experiment and =
it is in the state of evolution.  To make this new pilot successful, =
there is need for careful review by looking at all levels such as =
design, implementation and outcome and based on the lesson learnt by all =
key stakeholders including implementing NGOs, recommendations to be =
reviewed and if logical , adjusted into the implementation mechanism.=20

=20

Meanwhile, I am writing this letter of concern and disquiet on behalf of =
Sehatmandi (BPHS/EPHS) project implementers. This is a follow up of =
numerous letters submitted to MoPH and World Bank during the last couple =
of years, without any positive results and responses or providing =
acceptable feedback. The recent letter was signed by all NGOs, and =
raised once again concerns on management of Sehatmandi, breach in =
contract on force majeure and other issues, and imposing new conditions =
without proper consideration of contractual modality, country context =
and funding limitation.

The NGOs are concerned that with the current extension plan, new =
conditions and not considering their feedback, the quality-of-service =
delivery will deteriorate, and the patients and community will suffer =
due to inappropriate contractual arrangement and financial resources. I =
am attaching a few letters communicated to MoPH and World Bank, for your =
information.

In summary, the major issues have been as follow:

1.      During the current Sehatmandi contract implementation, new =
conditions are put forward which resulted into low resources and in =
contrast with the P4P concept. The letters of the ex minister are =
attached, e.g. on 20% payment. The extension period asks for new =
conditions, which is totally in contrast with the P4P concept, and it =
will severely affect the quality of care.

2.      Force majeure is a contractual requirement, where due to =
security and closure of health facilities and letter from MoPH during =
Covid-19 services are directed towards Covid-19 and stopping the =
services effected performance and it never compensated. It is seen as =
ignoring and breach of contract, where force majeure must be =
implemented. Covid-19 pandemic and lock down reduced the clients in =
quarter 2 and quarter 3 of 2020. It resulted into a significant =
reduction of service delivery and meeting the targets. Thus, the income =
for the project is reduced and in addition to that hike in prices and =
instructions of MoPH for prioritizing the services, aggravating the =
situation. NGOs did ask MoPH to not consider P4P for quarter 2 and =
quarter 3 of 2020, but so far, no response. The issue of force majeure =
and recommendation was part of mid-term review last year, and so far, no =
action, besides repeated requests from NGOs, has been taken. NGOs =
recently received a letter from MoPH to stop elective surgeries, where =
it is linked with payment, if this is stopped, from where the fund for =
salary of staff and cost of services will come, this is not considered.

3.      The payment of installments, linked with third party =
verification is delayed. Beside that the installment schedule is not in =
line with the expenses, and due to delays, it is not possible for many =
NGOs to pay on time salaries and other expenses. A revised installment =
schedule and on time verification and payment, is needed. This is not =
considered for the extension period.

4.      The extension budget is kept as it was during the last 30 months =
of Sehatmandi. The inflation rate, hike in prices and now condition to =
increase in salaries during the extension period, is not considered in =
the budget. With the same budget, it is not possible to run the =
services, due to above reasons, security issues and increased demands. =
This is requested by NGOs, no response from MoPH and World Bank, even =
though discussed during face to face and online meetings. It is =
disappointing, NGOs as for nonprofit, and as humanitarian body, not =
listened. Some national and international NGOs are forced due to above =
reasons and other reasons to withdraw and others considering the =
withdrawal if the environment and conditions are not feasible to =
implement the Sehatmandi during the extension. Anyone will implement, =
will be facing serious challenges and results into dissatisfaction of =
the community.

5.      Pharmaceutical=E2=80=99s quality is the responsibility of MoPH =
and this has to be ensured with improved quality control and license to =
import to the country. NGOs are blamed for quality but if =
MoPH/government provides the quality certificate, then they are =
responsible for it. Besides that NGOs have set targets, where payment is =
based on the targets defined, and thus the supply. NGOs request for =
proper target setting for extension period based on actual needs, to be =
able to supply enough pharmaceuticals. The issue of over-caps (ceiling =
for P4P) is important and besides many promises by MoPH we do not see =
any action so far. Without resolving the cap issues for indicators, it =
is not possible to resolve the quantity of pharmaceutical supplies.=20

6.      NGOs raised issues on third party monitoring reports, deductions =
issue, and SoP problems, which has to be considered and properly =
addressed during the extension.

7.      The collateral and direct damage to health facilities due to =
war, did require and will require a major rehabilitation work. This was =
not supported through Sehatmandi and is not foreseen in the extension =
period.

=20

We need to improve the sprite of BPHS/EPHS implementation and one-sided =
and adhoc decisions and actions, results into chaos and =
misunderstanding. NGOs, both national and international, have =
significant contributions and improvement in the health sector, =
mobilizing resources and developing trust of donors and the =
international community. NGOs need proper consideration, their voices =
are heard and achievements in health sectors due to them, should be =
recognized.=20

=20

NGOs can keep all health facilities functional, equipped and most of the =
supplies available, and face all challenges due to security and pressure =
from all directions. NGOs are proud of health sector achievements during =
the last two decades, and comparing to other sectors in Afghanistan, it =
has been a role model.=20

=20

I thus request on behalf of NGOs, government of Afghanistan, donors and =
supporting countries for hearing their concerns that are mentioned above =
and in the attached letters, and start dialogue with NGOs for current =
challenges, long term modalities of funding and implementation, as NGOs =
are in the field for last many decades and do understand the country =
context and needs.

stakeholders, if we want to improve the health system and healthcare in =

Subject: Open Letter of Serious Concern on Sehatmandi Project =
implementation in Afghanistan<o:p></o:p></div></div><o:p>&nbsp;</o:p>[External]<o:p></o:p><div><div>Dear =
Sir/Madam<o:p></o:p>NGOs played a vital role, for revitalization =
and sustaining an effective health system in Afghanistan during the last =
four decades and especially during the last twenty years. The =
contribution of NGOs, and closely working with MoPH and the trust =
developed by donors and community, enabled the health system to be a =
model approach in the country. This needs to be further built on, moved =
to the next level, and the opportunities to be used for the best of the =
country. The current Sehatmandi project was a new model, which is =
piloted in large scale, with positive results and sure with challenges. =
NGOs feel that this new modality of services delivery that is based on =
the principles of performance-based payment in the entire country, is a =
new experiment and it is in the state of evolution.&nbsp; To make this =
new pilot successful, there is need for careful review by looking at all =
levels such as design, implementation and outcome and based on the =
lesson learnt by all key stakeholders including implementing NGOs, =
recommendations to be reviewed and if logical , adjusted into the =
implementation mechanism. <o:p></o:p>&nbsp;<o:p></o:p>Meanwhile, I am =
writing this letter of concern and disquiet on behalf of Sehatmandi =
(BPHS/EPHS) project implementers. This is a follow up of numerous =
letters submitted to MoPH and World Bank during the last couple of =
years, without any positive results and responses or providing =
acceptable feedback. The recent letter was signed by all NGOs, and =
raised once again concerns on management of Sehatmandi, breach in =
contract on force majeure and other issues, and imposing new conditions =
without proper consideration of contractual modality, country context =
and funding limitation.<o:p></o:p>&nbsp;<o:p></o:p>The NGOs are =
concerned that with the current extension plan, new conditions and not =
considering their feedback, the quality-of-service delivery will =
deteriorate, and the =
patients and community will suffer due to inappropriate contractual =
arrangement and financial resources. I am attaching a few letters =
communicated to MoPH and World Bank, for your =
information.<o:p></o:p>&nbsp;<o:p></o:p>In summary, the major =
issues have been as follow:<o:p></o:p>1.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; During the current Sehatmandi contract =
implementation, new conditions are put forward which resulted into low =
resources and in contrast with the P4P concept. The letters of the ex =
minister are attached, e.g. on 20% payment. The extension period asks =
for new conditions, which is totally in contrast with the P4P concept, =
and it will severely affect the quality of care.<o:p></o:p>&nbsp;<o:p></o:p>2.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Force majeure is a contractual requirement, =
where due to security and closure of health facilities and letter from =
MoPH during Covid-19 services are directed towards Covid-19 and stopping =
the services effected performance and it never compensated. It is seen =
as ignoring and breach of contract, where force majeure must be =
implemented. Covid-19 pandemic and lock down reduced the clients in =
quarter 2 and quarter 3 of 2020. It resulted into a significant =
reduction of service delivery and meeting the targets. Thus, the income =
for the project is reduced and in addition to that hike in prices and =
instructions of MoPH for prioritizing the services, aggravating the =
situation. NGOs did ask MoPH to not consider P4P for quarter 2 and =
quarter 3 of 2020, but so far, no response. The issue of force majeure =
and recommendation was part of mid-term review last year, and so far, no =
action, besides repeated requests from NGOs, has been taken. NGOs =
recently received a letter from MoPH to stop elective surgeries, where =
it is linked with payment, if this is stopped, from where the fund for =
salary of staff and cost of services will come, this is not =
considered.<o:p></o:p>&nbsp;<o:p></o:p>3.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The payment of installments, linked with =
third party verification is delayed. Beside that the installment =
schedule is not in line with the expenses, and due to delays, it is not =
possible for many NGOs to pay on time salaries and other expenses. A =
revised installment schedule and on time verification and payment, is =
needed. This is not considered for the extension =
period.<o:p></o:p>&nbsp;<o:p></o:p>4.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The extension budget is kept as it was during =
the last 30 months of Sehatmandi. The inflation rate, hike in prices and =
now condition to increase in salaries during the extension period, is =
not considered in the budget. With the same budget, it is not possible =
to run the services, due to above reasons, security issues and increased =
demands. This is requested by NGOs, no response from MoPH and World =
Bank, even though discussed during face to face and online meetings. It =
is disappointing, NGOs as for nonprofit, and as humanitarian body, not =
listened. Some national and international NGOs are forced due to above =
reasons and other reasons to withdraw and others considering the =
withdrawal if the environment and conditions are not feasible to =
implement the Sehatmandi during the extension. Anyone will implement, =
will be facing serious challenges and results into dissatisfaction of =
the community.<o:p></o:p>&nbsp;<o:p></o:p>5.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Pharmaceutical=E2=80=99s quality is the =
responsibility of MoPH and this has to be ensured with improved quality =
control and license to import to the country. NGOs are blamed for =
quality but if MoPH/government provides the quality certificate, then =
they are responsible for it. Besides that NGOs have set targets, where =
payment is based on the targets defined, and thus the supply. NGOs =
request for proper target setting for extension period based on actual =
needs, to be able to supply enough pharmaceuticals. The issue of =
over-caps (ceiling for P4P) is important and besides many promises by =
MoPH we do not see any action so far. Without resolving the cap issues =
for indicators, it is not possible to resolve the quantity of =
pharmaceutical supplies. <o:p></o:p>&nbsp;<o:p></o:p>6.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; NGOs raised issues on third party monitoring =
reports, deductions issue, and SoP problems, which has to be considered =
and properly addressed during the extension.<o:p></o:p>&nbsp;<o:p></o:p>7.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The collateral and direct damage to health =
facilities due to war, did require and will require a major =
rehabilitation work. This was not supported through Sehatmandi and is =
not foreseen in the extension period.<o:p></o:p>&nbsp;<o:p></o:p>We need to improve =
the sprite of BPHS/EPHS implementation and one-sided and adhoc decisions =
and actions, results into chaos and misunderstanding. NGOs, both =
national and international, have significant contributions and =
improvement in the health sector, mobilizing resources and developing =
trust of donors and the international community. NGOs need proper =
consideration, their voices are heard and achievements in health sectors =
due to them, should be recognized. <o:p></o:p>&nbsp;<o:p></o:p>NGOs can keep all =
health facilities functional, equipped and most of the supplies =
available, and face all challenges due to security and pressure from all =
directions. NGOs are proud of health sector achievements during the last =
two decades, and comparing to other sectors in Afghanistan, it has been =
a role model. <o:p></o:p>&nbsp;<o:p></o:p>I thus request on =
behalf of NGOs, government of Afghanistan, donors and supporting =
countries for hearing their concerns that are mentioned above and in the =
attached letters, and start dialogue with NGOs for current challenges, =
long term modalities of funding and implementation, as NGOs are in the =
field for last many decades and do understand the country context and =
needs.<o:p></o:p>&nbsp;<o:p></o:p>Do look forward to a =
positive response and consideration by all stakeholders, if we want to =
improve the health system and healthcare in the =
