Greeting

Today we had meeting with HQ and one of our agenda was about trauma =
project closing preparation for external audit, therefore below document =
need to be prepare and send us before 24 /April/2021 as following;

1.       HMIS monthly  Report from start of project up to now.

2.       Monthly narrative report.

3.       All Monitoring and supportive supervision report from start of =
project up to now=20

4.       follow up action plan of Monitoring and supportive supervision =
finding feedback with progress of each one.

5.       Coordination meeting with all stakeholder (PHCC meeting and =
ect.).

6.       Proposal of Trauma project.

7.       MoU of Trauma project, JIA with PPHD and JACK.

8.       Community score card report

9.       Providing of presentation from start  up to  end of the project =
which including all of the project achievement base on project work plan =
and project goal which are affected the service delivery in targeted =
population.

Note; All the above documents should be  stumped , singed and submit the =
scan copy to main office. The original documents should be recording to =
the file in Provincial office.
