We have deployed 2 RRTs and 5 district center focal points up to now. =
Their performance is good, however it needs to get improved. MoPH and =
PPHD is giving us feedback on RRTs and DCs performance each and every =
day. I have gone through challenges and am writing them in below lines: =
(although we need to discuss it in a Coordination meeting, here I will =
only write them down)

=20

=C2=B7         According to decision on Thursday 14th of January 2021 by =
MoPH in video conference, every district center should take samples. =
RRTs will take samples but not in health facilities, DCs must take =
samples in Health facility they are. Our district center officers are =
MDs and they are not trained on sample collection. They can infect =
themselves or even their sample collection may not be effective at all. =
Our DCs are in DHs and CHCs, there are lab technicians in each CHC and =
DH. According to Contract, ERHSP project should not affect negatively on =
Sehatmandi project, however if lab technicians of CHCs and DHs help in =
sample collection and it does not affect negatively on Sehatmandi =
project, we will be in safe zone from critics of PPHD and MoPH.

=C2=B7         ERHSP project currently does not have any ambulance =
contracted, however if RRTs, DCs and any health facility report a severe =
or critical case of COVID-19 outside the Kunduz city, how should we =
transfer them to COVID-19 hospital? Although ERHSP project activities =
should not affect negative on Sehatmandi project, but we do not have any =
other choice rather than to use ambulances of Sehatmandi project. We =
should decide a best way to use them and not affect negatively on =
Sehatmandi project.

=C2=B7         We only have 9 DCs and there are 75 Health facilities. =
Excluding 28 Sub health centers, other health facilities should have a =
direct focal point for COVID-19 suspect cases. The best choice for this =
option is CHS of each health facility. According to Contract of ERHSP =
project, CHW should help in contact tracing, case finding and awareness, =
CHS can coordinate these activities and effectivey monitor the process =
by CHWs in community level and by himself in Health facility level. In =
28 Sub health centers, Health facility in charge should be responsible =
for COVID-19 suspect case report to RRTs. We only have 5 RRTs and 9 DCs =
and they cannot cover all 75 HF. My recommendation is that after =
decision on this, an official letter should be issued and cascade to all =
HFs for better supporting of RRTs.

=C2=B7         As we all know contact tracing is the most important part =
of ERHSP project, however it is only possible by passive surveillance. =
Passive surveillance is done by CHWs, CHSs and health facility in =
charges (SHCs). We have to make an action plan for passive surveillance =
in order to achieve the goal of active surveillance.=20

=C2=B7         There are 371 cases not traced (some from first round of =
ERHSP project). I will strictly follow them and will report back the =
activity by 20th January 2021. I might need help from HMIS department. =
If we do not trace these cases before Wednesday, MoPH might send us a =
letter officially because we are the only province in Afghanistan having =
371 active cases.=20

=C2=B7         Dr Majid hassan has noted down the documents needed by =
TPM. We will arrange our documents in second round in accordance with =
TPM checklist. I will need help of Dr Sahib Majid Hassan and I am sure =
he is fully supportive as always (Thanks).

=C2=B7         I would like to honestly admit that in Contract and =
Technical proposal of ERHSP project, only one  part was non =
understandable for me. The indicator percentage, how they are =
calculated. I would like to kindly request for an orientation on these =
indicator percentage.=20
