Dear BPHS/EPHS Implementing Partners,

The Ministry of Public Health (MoPH) would like to conduct a short =
survey on user experience with the Harmonized Quality Improvement =
Program (HQIP tools). The survey will take about 10 -15 minutes to =
complete. We greatly appreciate your participation in this survey.  We =
expect at least two relevant technical staff; 1 from head office and 1 =
from provincial team. Your responses will help us to strengthen efforts =
to improve the quality of maternal, newborn, and child health services.=20

The deadline to complete this online questionnaire is COB Wednesday, 02 =
DEC, 2020 ONLY.

The purpose of this survey is to collect the experiences of health care =
managers and service providers with the HQIP tool, to understand what =
was most appreciated by health providers/managers and what, if anything, =
they would change about the tool.
