Subject: Replacement of 1 guard/cleaner in Qotor Bolaq BHC FATP

Dear Amin Jan

1 guard/cleaner from Qotor Bolaq BHC FATP named Mohammad Essa was absent
from 1st of November 2020 until now. According to the needs of FATP,
community recommendations and JACK HR policy, guard/cleaner is needed in
Qotor Bolaq BHC FATP. So the community members along with approval of AOG
health commission responsibles has introduced Ghulam Farooq son of Ghulam
Rasool. He is from the same area of the FATP so he can work smoothly.

These were recommendation from community, however kindly apply HR policy and
update me on the process.
