Monitoring Plan=E2=80=9D sheet, for our respective program managers to =
share the supervision and monitoring activities.

Also I have shared the =E2=80=9Cchecklist-COVID-19.pdf=E2=80=9D file and =
also =E2=80=9CCOVID-10 health facility Checklist Database=E2=80=9D

As you know one of the COVID19-EHSSP objective is =E2=80=9CPercentage of =
HF comply with IPC Protocol=E2=80=9D which will be verified by TPM (3rd =
Party monitoring report) so we have to make sure in our all health =
facilities in Kunduz province and all screening site.

So also our colleague, in all COVID-19 project and other project, could =
apply in their sites and area, this checklist or any other tools for =
their supervision and monitoring.

So my request is from all AHF-COVID-19 and COVID19-EHSSP project (Dr. =
Mashal, Dr. Miakhil and Dr. Saqib) to put their supervision and =
monitoring activities done during project period, and next plan the =

Following are the findings from Emergency lifesaving Intervention to =
prevent COVID-19 amongst high risk population in Kunduz province project =
; please follow one by one with relevant project manager and involve the =
provincial manager too; and share with me the plan and after the =
deadline the progress plan a long with the supportive documents;

1.       Less monitoring and supervision conducted from the screening =
sites=20

2.       The performances are very low with comparison fo the target=20

3.       The supply to screening sites is very irregular and even there =
isn=E2=80=99t anything in the screening sites and all the materials are =
stocked in the warehouse=20

4.       Mulla Quli screening site has not visited yet=20

5.       Hygiene , and disinfection materials were not in the screening =
site

6.       IEC materials were not supplied

7.       CHWs were not checked whether they received the materials of =
COVID-19 or not

8.       Huge discrepancy between the reports and registers=20

9.       Low knowledge of the staff in the screening sites , they did =
not know most of the activities=20

10.   The screening sites head nurse did not know even the registration =
of the patients=20

11.   Awaz posters were not distributed=20

12.   Targets were not printed to screening sites=20

13.   BoQs of ambulances were not in the ambulances=20

14.   There was not any visit book in the screening site

15.   The screening site was disarrange and needed for cleaning by the =
cleaner=20

16.   Still we are 42% in budget line of To-up card

17.   We are 42% in HMIS tools printing and we have enough printed =
materials so please follow this with logistic and finance that till end =
of the project this line must be zero and to be spent all.

18.   Majority of the CHWs and other materials were in the warehouse and =
they must not be in the warehouse , because they must be supplied .
