The mission which was conducted on 8-14 = December 2019 the improvement plan is prepared and all the tasks which = were related to JACK are conducted and reported done, but except two = tasks are partially or completely in pending which are related to SCI; = and the Improvement plan is attached to you, please kindly find it. The = supportive documents for each tasks are existed and based on request we = will provide to you.=20 =20 And regarding the recent mission which was conduct on 19-26 August 2020 = ; the Improvement Plan is prepared and it is attached too, please kindly = find it and following are some more justification and improvement on the = feedback provided.=20 =20 - Frequent turnover of the project coordinator=20 Justification and Improvement: In the start of the project according to = the plan project coordinator recruited and he worked for 11 months and = then he resigned due to salary issue; and we recruited another person = he also resigned after 4 months and then we recruited another person he = also resigned due to salary issue; then for the fourth time we announced = the position and after complete search and discussion with candidate = that till end of the project you are not allowed to resign from the = position and we took a guarantee letter too from him, except in some = situation which are mentioned in contract such as contract obligation. = Now the project coordinator is recruited and he is well involved in the = project.=20 - Near to expire pharmaceutical list was not available to guide = the team to prepare the distribution plan for the near to expiry items = before its expiration. Two items were about to expire (end of September = 2020) with very huge quantity: 1. 6,280 bottles of Paracetamol 120 mg = syrup; 2. and 6,000 bottles of Co- amoxiclav 156.25 mg syrup. = Additionally, 40,100 zinc tablets will be expired on November 2020. =20 Justification and Improvement: =20 The list of near expire was physically shared with SCI provincial office = by our warehouse officer . And the plan for distribution is prepared and = all the near to expire medicines will be consumed. =20 =20 - 3,840 Charcoal tablets, and 20 Lidocaine ampules were available = in pharmaceutical stock with zero average monthly consumption. It is = most likely these items will remain in stock till end of the project = with no use. =20 Justification and Improvement: =20 According to the Mobile Health concept the charcoal tablets and = lidocaine is not consumable , because it need proper room and = hospitalization preparation; so the list of the medicines is shared with = SCI provincial office to take proper decision.=20 - Since start of the project there were no evidence of physical = inventory of pharmaceuticals and medical consumables. Justification and Improvement: =20 For few months in the start of the project they physical inventory was = not considered, but now it is considering regularly and the documents = are available a long with reports.=20 - The new enrollment of MAM and SAM children were limited that led = to over stocking of RUSF (478 cartons) in the medical warehouse of JACK. Justification and Improvement: =20 Since the start of the project SCI colleagues mentioned to our = provincial team that beyond the target the beneficiaries should not be = considered for all indicators , because the medicines and RUSF and other = needed materials will not be supplied; but now after the feedback which = we received from SCI team it is ordered to teams that not be limited to = the target and cover beyond the target too.=20 - Pharmaceutical were miss placed in pharmaceuticals shelves = almost 3-meter-high form the floor. Syrups were placed at the top of = shelves which have the risk of damage (fall down) if the shelves are = shacked.=20 Justification and Improvement: =20 In these days we had overall BPHS health facilities medicines supply so, = the stock was a little disarrange but the stock management orientation = conducted to the relevant staff and all the medicines and = pharmaceuticals are settled according to the stock management = guidelines.=20 - No rental vehicle for supportive supervision of BMZ supported = mobile health teams by JACK project staff (Budget is allocated for = this).=20 Justification and Improvement: =20 For synergism among the projects there are many vehicles in Sehatmandi, = trauma and other projects so the vehicle is there, but we will follow = very seriously to completely separate the BMZ vehicle , but beyond that = the other projects vehicles are at their services too. And the = supportive supervisions are going normal.=20 - The project staff is not attending relevant provincial meetings = to represent the project, and EPI report is not submitted to DoPH for = six months. =20 Justification and Improvement: =20 As there was gap the availability of the project coordinator , it was = the reason that in some meetings our representatives were not there, but = now according to the plan regularly the project coordinator is attending = all the meetings.=20 - National nutrition online database was not updated for SAM and = MAM reports. Justification and Improvement: =20 In term when the project coordinator was not recruited , it was delayed = but now it is regularly entered and this is followed.=20 =20 I am on mission in Kunduz and I am following all the above mentioned = issues very seriously and the details another progressive report a long = with supportive documents will be shared with you. And the reporting = flow is improved we share monthly statistics and narrative report with = both provincial and main offices of SCI and also the quarterly technical = progressive report share too.=20 =20 Coordination meetings will be more increased with all stakeholders such = as DoRR, PPHD, MSF, HI, UN-OCHA, WHO, UNICEF, Directorate of labor and = social affairs, particularly for BMZ project and the coordination = reports will be shared with you too.=20 =20 We created a mechanism of project activities follow up, which is weekly = basis online meeting between provincial office and main office to = discuss the project improvement and progression.=20 =20 There are many success stories regarding the project which this project = has the positive impact in the community and people accepted it with = positive view.
