Dear Dr. Sahib Basharat
Hope all is well with you and your respected family
Base on my supportive supervision I have found the following important poin=
ts that I want to share with you for your attention and improvement.
Findings:
1.            Vaccine services delivery stopped due to vaccinator absence f=
rom 21 - 26 July 2020 in team B
2.            Daily usually (more than 3 staff ) are in left, it will cause=
 to stop some components of  MHT services
3.            Usually, staff taking leave by call without an official leave=
 request form.
4.            Staff need HMIS refresher Training
We are all waiting for your response and improvement and also it will be be=
tter to pay this month's MHTs staff salary before the Eid ceremony to cover=
 their needs.
