Dear Amin Jan,

Base on the report of ME from Anghorbagh BHC , the health facility staff
were absent after 02:00 pm. Today we followed it myself and had meeting with
the staff in this HF. Bellow decisions were got there:

1 -one official warning for all the Staff  (just Head of clinic who was in
leave).

2-last warning for the clinic guard.

So it is shared with you and if any of our management team don't have any
comment please prepare the warning letter and send it to the HF, also keep
in their personal file.
