Based on the MoPH leadership instruction, a team from MoPH has been assigned
to review the Financial Management System of your organization due to delays
on payment of health facility staff salaries and supply of medicine.

Please be informed that that the team will visit your main office tomorrow
Wednesday July 15,2020 at 9:00 am.

Therefore, you are requested to prepare the below documents in advance for
our investigation:

1.	Detail list of payments made from the 4th installment of lump sum
and 4th installment of P4P 
2.	List of all contracts (goods, services and rehabilitation) for FY
1398 and 1399 and the status and % of their payments 
3.	Bank Statement of the project (From the start of the project to
date)
4.	HFs Payrolls for Hamal, Sawr and Jawza 1399
5.	Bank Statement (which shows salary transferred to HFs staff) from
the start of the project to date.
6.	Asset Registrar (Cumulative to date)
7.	List of purchased pharmaceutical, effective from start of Sehatmandi
project
8.	Supply/distribution list of medicine to the project from start of
Sehatmandi project
