Subject: Imam Sahib DH ambulnce accedent Report.

Dear Dr. Sahib Ghani,

Salam and hope you are fine, than based on the report of HC  of Imam sahib
DH on  accident of its ambulance with a motor bike which was happened in
this city yester. During of the event one person was died and the second was
seriously injured and referred to Kunduz RH. Our ambulance driver is
arrested with police and the ambulance is not working properly and is
damaged ( Mr. Shafiq can share the picture).

So based on the decision of the officer we as a team (Dr.Musa,Mr.Tanha and
Mr.Shafiq) had visit from the health facility to follow it . unfortunately
due to the security situation we could not have meeting with district
governor and discussed with him on this regards.

Also there was active war in the past night in Imam Sahib district and 6
military died and 3 injuries were brought to the hospital , services were
provided for them . we had meeting with the DH staff and discussed on the
challenges of the referral patients and decide to use the ambulance of Basos
CHC until it is needed and the DH ambulance is allowed to be repair and
used.

So you are kindly requested to share the issue with related authorities of
JACK and waiting for your guidance on this regards.
