As per the contract obligations JACK is delivering the service according =
the guidelines and policies of MoPH, as you know the high amount =
deduction is from under-five OPD indicator where we have already raised =
our concerns in this regard that the register does not include father =
name of child in column which was one the major challenge that TPM =
couldn=E2=80=99t succeed to verify the cases in the field and in other =
hand, it=E2=80=99s clear to all Kunduz is one of the most insecure =
province with high number of conflicts which caused displacement of =
families inside and outside of the province. If we refer to the TPM =
figures this will be clear that in other Under-five indicators such =
Penta and IYCF JACK have achieved more than 90% then how could be =
possible that we achieve less than 15% in under-five OPD indicator. =20

As you know, deduction of this amount not only affect health workers =
high rate of turnover and their demotivation but it also effect on =
quality of service provision in a such conflict and insecure area. In =
order to avoid any kind of interruption and poor service delivery to the =
needed population of Kunduz, we request MoPH/GCMU to consider this issue =
in such exceptional situation and advise us ahead.=20

We reviewed email from JACK as well as our internal data.

All data for under 5 morbidity indicator from TPM is correct (IP does =
not claim data would be incorrect).

The issue is that TPM team could identify only few patients on this =
indicator in Kunduz.
When TPM cannot identify a patient on their own, survey team comes back =
to HF and ask for support in identification of patient (or otherwise HF =
has to confirm that patient could not be identified, as per to be signed =
and stamped community verification form).
This is to ensure IPs do have the chance to participate and do not get =
penalised without reason.

IP in Kunduz relates low number of identified patients to the standard =
HMIS report tool structure (i.e. lack of column for name of patient's =
father).
While this is indeed not ideal, IPs in other provinces usually simply =
modified the registers and created space or made a slash (/) between the =
name and father name.
Each IP should be able to identify their patients and this should be =
reflected in IPs internal verification system (see also SOP on this).

It is beyond the remit of TPM to advise on the claim and request of IP =
of Kunduz, as the question and concern is not related quality of TPM =
data as such.
The decision is solely at the discretion of MoPH.
