Dear All supervisor :

First of all hope you are fine , then based on the meeting we had in our
office and  as you know we are in the last week of Sawr 1399 and The Eid of
Feter is coming in next week, so please inform your related Health
facilities to prepare their monthly HMIS reports on 29  and submit on 30 the
sawr 1399.

So from today have communication on this issue and the reports should be
completely collected .

Dear Dr.Sahib Majeed,

Please prepare the  list HMIS reports as a checklist for ech supervisor to
collect the health facilities reports completely and one should be missed .
if you have any commitments please let me know .

Note: all the reports should be prepared in two original copies in the
health facilities , one for HMIS and the second should be saved in the
health facilities.
