Just for Afghan Capacity and Knowledge is BPHS implementer in Kunduz =
since January 2019, during this period JACK had tangible and remarkable =
progression in delivering qualified health services to people. JACK =
built and maintain equilibrium of coordination , communication and =
cooperation with all stakeholders, such as Kunduz governor office, PPHD, =
and all other national , international NGOs and UN-bodies.  Although we =
were busy with hand/take over process in first two months of the =
project, beside that  we have good performance according to the =
contracted P4P indicators, even more than three indicators are touched =
the maximum and there isn=E2=80=99t any indicator near or touch the =
minimum and we hope second quarter would have more achievement.=20

=20

Reference to our face to face meeting held in GCMU regarding recent MoPH =
missions , the following justifications provided based on findings,=20

=20

I.                 Justifications on unavailability of Key Staffs:

=20

=C2=B7       Deputy Technical Manager:

=20

Deputy Technical manager in proposal named Dr. Sayed Amanullah Zahid =
started his job with JACK from January 2019 up to April 2019 and then he =
resigned on 01, April, 2019 effective date till 30, April, 2019, the =
reason of his resignation was his personal and family issues and his =
resignation letter shared with GCMU, then the position announced and the =
second candidate introduced to GCMU and his documents analysis still =
under process with GCMU.

=C2=B7       Deputy Finance Manager:

Deputy finance manager his name Mr. Obaidullah Shahzad is working with =
JACK since 01, January, 2019 up to now and his present in his position =
in province.

For further clarifications please refer to Annex-I

II.               Justifications on (Stopping of Laboratory Service in =
BHCs):

There was a vertical project of malaria in Kunduz, and that project were =
doing malaria activities, so in some BHCs microcopists were recruited =
and in that time Kunduz was high risk of malaria, but after launching of =
(The Strengthening and Scaling-up Malaria Prevention and Case Management =
to improve Health Status in Afghanistan) funded by Global and under PR =
responsibilities so UNDP since 2016 in Kunduz with the following =
objectives;

=20

=C2=B7       To reduce malaria morbidity by 85% in all malaria stratum =
by the end of 2017;

=C2=B7       To reduce malaria mortality to 0 by the end of 2017;

=C2=B7       To reduce the incidence of P. falciparum malaria to =
sporadic cases by the end of 2017 with a vision to completely interrupt =
transmission of P.f by 2020;

=C2=B7       To reduce malaria morbidity in high risk area (stratum one =
districts) by 80% by the end of 2017.

And updated in 2019 the above mentioned  objectives;

This project implementation is with the following mechanisms;=20

=20

Case Management:

=C2=B7       Three days training regarding malaria diagnosis by RDT, =
treatment according to NTG and reporting for in charge of health =
facility (BHCs, SHCs & MHTs)

=C2=B7       Two days training regarding malaria diagnosis by RDT, =
treatment according to NTG and reporting for CHS (Initial and refresher)

=C2=B7       Two days training regarding malaria diagnosis by RDT, =
treatment according to NTG and reporting for CHW (Initial) in malaria =
strata 3 and 4 districts

=C2=B7       One day training regarding malaria diagnosis by RDT, =
treatment according to NTG and reporting for CHW (Initial) in malaria =
strata 1 and 2 districts

=C2=B7       Five days training regarding malaria diagnosis by =
Microscopy and reporting for lab technicians in strata

=C2=B7       Diagnosis of PF, PV and Mix through RDTs, on BHCs, SHCs, =
and even HPs level.

=C2=B7       Treatment of detected cases.=20

=20

Vector control:

=C2=B7       LLINs distribution through health facility for pregnant =
women in strata 1, 2a and 2b districts (pregnant women well receive =
LLINs only in their first ANC visit in the health facility).Only in =
Kandahar center district.

So, looking to the above mentioning there isn=E2=80=99t needed =
laboratory detection of malaria cases in BHCs and instead RDTs are using =
as well as minimum standard of new P4P concept based on SOP considered. =
And according to your request two previous years=E2=80=99 performances =
of relevant BHCs lab attached.=20

III.             Stationary and Hygiene Supply:

=20

Stationery and hygiene supplied for first quarter 2019 January, February =
and March 2019 and also for April, May and June, 2019 too. Currently HFs =
have adequate stationery and hygiene in HFs.=20

IV.             Hand and Take Over:

OPHM was BPHS implementer in Kunduz province in 2018 and since January =
2019 JACK Sehatmandi project implementer under BPHS program in Kunduz =
province. There were some challenges in hand/take over process, the =
issue has had shared with PPHD of Kunduz province, after usually follow =
up, finally a delegation came from OHPM main office for the finalizing =
of hand/take over issue of Kunduz BPHS program. And the hand/takeover of =
the following issues conducted,

=20

=C2=B7         The expenses for electricity, gas of vaccines, credit =
cards of staff, bills of HFs and pending salaries paid.

=C2=B7        Essential kits for 25 HPs which includes shelfs and boxes =
purchased.

=C2=B7        Medical and non- medical equipment (computer, anesthesia =
machine, cardiac monitor. etc) which were transferred to Kabul have been =
returned back to Kunduz.

After hand/takeover of above mentioned issues, the report prepared and =
signed by representatives of both present and previous BPHS =
implementers, representative of PPHD, representative of economy =
directorate, Mastofait representative and Kunduz office governor =
representative and finally report shared with Kunduz PPHD. Meanwhile =
inventory list of office, CME/CHNE school and HFs were signed and =
approved in five copies. The copies shared with the following =
departments;

1.     PPHD

2.     JACK

3.     OHPM

4.     Mastofiat

5.     Economy Directorate=20

=20

V.               Medicines supply:

First quarter January, February and March 2019 medicines supplied in the =
start of the project and for April, May and June, 2019 supplied to =
province and to all HFs supplied some before Eid and some after Eid , so =
currently all HFs have medicines the quantity and quality considered in =
medicines supply. The medicines purchasing and supplying issues =
considered according to the Afghanistan procurement law and quality =
control of MoPH policies.=20

=20

VI.               CME/CHNE needed items:

After mission of GCMU monitor from CME/CHNE school of Kunduz, some items =
and materials purchased for CME/CHNE such as cabinet, exas for =
ventilation and some other.=20

high risk of malaria, but after launching of (The Strengthening =
and Scaling-up Malaria Prevention and Case Management to improve Health =
Status in Afghanistan) funded by Global and under PR responsibilities so =
UNDP since 2016 in Kunduz with the following =
